摘要

目的

研究颞叶癫痫患者的认知功能状况及其与中医体质类型的相关性。

方法

纳入首都医科大学宣武医院神经内科病房和中医科门诊确诊的颞叶癫痫患者98例，与正常组30例进行比较。观察蒙特利尔认知评估量表(Montreal Cognitive Assessment，MoCA)评分和中医体质分布。

结果

颞叶癫痫患者的MoCA评分总分和视空间执行力、注意力、语言能力、延迟记忆能力子项目评分低于正常组(

0.01)。颞叶癫痫组较正常组相比气虚质和湿热质的患者多于其他体质(

0.05)；颞叶癫痫患者中认知正常者平和质多于其他体质(

0.01)，合并认知障碍者气虚质多于其他体质(

0.05)。

结论

颞叶癫痫患者的认知功能降低明显，体现在视空间执行力、注意力、语言能力、延迟记忆能力等方面。从中医的9种体质比较，颞叶癫痫气虚质和湿热质患者多见，颞叶癫痫气虚质患者更易合并认知障碍。

Abstract

Objective

To observe the cognitive function in patients with temporal lobe epilepsy (TLE)

and to explore the correlation between TCM constitution types and TLE and their cognitive function.

Methods

A total of 98 TLE patients were enrolled for cognitive tests and TCM constitution types tests

from the neurology ward and outpatient of TCM Department at Xuanwu Hospital of Capital Medical University from October 2018 to January 2020. Montreal Cognitive Assessment (MoCA) scores and TCM constitution types between the TLE group and normal group(

=30) were observed.

Results

There were significantly lower scores in MoCA and visuospatial and executive

memory

attention

language in patients with TLE(

0.01). As compared with the normal group

patients with the constitution of qi deficiency and with the dampness-heat constitution outnumbered those with other types in the TLE group (

0.05). In the TLE group with normal cognitive ability

patients with the peaceful constitution outnumbered those with other types(

0.01); in the TLE group with the cognitive impairments

the qi deficiency constitution was more than other types(

0.05).

Conclusions

The cognitive function of patients with TLE was significantly reduced

which was reflected in visuospatial and executive ability

attention

language ability and memory. From the comparison of the nine TCM constitutions

qi deficiency and dampness-heat constitutions are more common in patients with TLE

and qi deficiency patients are more likely to suffer cognitive impairments.
