摘要

慢性前列腺炎是男科常见疾病，由于其病因不明、症状复杂、病程迁延，增加了临床治疗难度。目前，立足脾胃后天之本濡养诸脏腑的生理特点，从脾胃虚衰角度来解释本病病因病机和指导治疗成为临床研究的新方向。随着肠道菌群的研究不断深入，肠道菌群与脾胃和慢性前列腺炎的关系被逐渐揭开，如肠道菌群在生理功能、致病因素、症状表现、病理状态以及药物调节方面与脾胃相似，慢性前列腺炎在病名症状、病因病机、治法方药上均与脾虚密切相关，且肠道菌群可借助神经、内分泌、免疫调节来诱发慢性前列腺炎。基于三者的相互联系，故认为脾虚导致肠道菌群失调进而诱发慢性前列腺炎，这不仅有益于解释慢性前列腺炎的发病机制，更可作为慢性前列腺炎从脾胃论治的科学支撑。

Abstract

Chronic prostatitis (CP) is a common disease in urology and andrology

which is a tricky disease due to its unknown etiology

complicated symptoms and prolonged course. At present

it has become a new direction of clinical research to explain the etiology

pathogenesis and treatment of this disease from the perspective of deficiency of the spleen and stomach based on the physiological characteristics of the two as the foundation of acquired constitution to nourish all viscera. With the research advancing

the relationship between gut microbiota and the spleen and stomach and CP has been gradually uncovered. For example

gut microbiota is similar to the spleen and stomach in physiological function

symptom manifestations

pathological state and medication regulation. CP is closely related to spleen deficiency in terms of disease name

symptoms

pathogenesis

treatment and prescriptions. Moreover

gut microbiota can induce CP through nerve

endocrine and immune regulation. Based on the relationship between the three

we believe that spleen deficiency leads to gut microbiota imbalance and thus induces CP

which can not help explain its pathogenesis

but also be used as scientific support for treating the disease from the perspective of the spleen and stomach.
