      摘要:ObjectiveTo observe the clinical efficacy of externally used Huoxue Sanjie Formula in improving autologous arteriovenous fistula in patients with maintenance hemodialysis.MethodsNinety-eight patients with maintenance hemodialysis were assigned to the treatment group and the control group according to whether they used externally Huoxue Sanjie Formula. There were 40 left in the treatment group and 52 left in the control group, with the withdrawals excluded. The control group was treated with routine protection measures, and the treatment group was additionally treated with Huoxue Sanjie Formula. The primary patency loss of autologous arteriovenous fistula, the occurrence of autologous arteriovenous fistula complications, the situation of seeking medical treatment or hospitalization due to fistula problems, the primary patency rate and the primary patency time of autologous arteriovenous fistula in the two groups were observed, and the related factors that may affect the primary patencyth of internal fistula were analyzed by single factor and multi factor analysis.ResultsThere was no significant difference in the primary patency loss of internal fistula between the treatment group and the control group at 6th month follow-up (P>0.05). At the 12th month and 18th month follow-ups, the primary patency loss rate of autologous arteriovenous fistula was lower in the treatment group than in the control group (P<0.05). As of the last follow-up, there was no significant difference in the incidence of autologous arteriovenous fistula complications between the treatment group and the control group (P>0.05). At the 6th month follow-up, there was no significant difference between the treatment group and the control group (P>0.05). At the 12th month and 18th month, the rate of seeking medical treatment or hospitalization due to fistula problems was lower in the treatment group than in the control group (P<0.05). Log rank test showed that the primary patency rate of internal fistula was higher in the treatment group than in the control group, and the primary patency time was longer in the treatment group than in the control group (P<0.05). The results of single factor analysis showed that phosphorus ≥1.8 mmol/L, plasma albumin ≥40 g/L, externally used Huoxue Sanjie Formula and hypotension during dialysis on the primary patency of autologous arteriovenous fistula were statistically significant (P<0.01). Multi factor analysis showed that hypotension during dialysis was an independent risk factor for the primary patency loss of autologous arteriovenous fistula, and externally used Huoxue Sanjie Formula and plasma albumin ≥ 40 g/L were independent protective factors for the primary patency loss of autologous arteriovenous fistula.ConclusionsExternally used Huoxue Sanjie Formula has a good clinical efficacy in improving the function of autologous arteriovenous fistula in maintenance hemodialysis patients.

      摘要:ObjectiveTo observe the clinical efficacy of modified "Old Ten Needles" therapy in patients with post circulation ischemic vertigo of the Qi and blood-deficiency pattern.MethodsSeventy patients with posterior circulation ischemic vertigo of the Qi and blood-deficiency pattern were randomly assigned to the treatment group (n=35) and the control group (n=35). The control group received basic treatment based on patients’ general conditions, for instance, blood pressure control, anti-platelet aggregation, lowering blood lipids, stabilizing plaques, adjusting blood sugar, improving cerebral circulation, thrombolysis and other conventional treatments. Apart from the basic treatment that the control group received, the treatment group received modified "Old Ten Needles" therapy. The clinical efficacy, traditional Chinese medicine (TCM) syndrome score, dizziness disorder inventory (DHI) score, average blood flow velocity of the left and right vertebral arteries and basilar artery were observed, and its safety was evaluated.ResultsAs for the clinical efficacy, 18 cases were clinically controlled, 8 cases were markedly effective, 7 cases were effective, and 2 cases were ineffective, with a total effective rate of 94.3% in the treatment group. In the control group, 7 cases were clinically controlled, 8 cases were markedly effective, 9 cases were effective, and 11 cases were ineffective, with a total effective rate of 68.6%. Nonparametric test showed that as compared with the control group, the modified "Old Ten Needles" therapy had advantages in clinical effects (Z=-3.25, P=0.001, P<0.05), indicating that the clinical efficacy was better in the treatment group than in the control group. The scores of TCM syndromes were significantly decreased after treatment as compared with before treatment in both groups (P<0.05). There was no significant difference between the two groups before treatment (P>0.05). After treatment, the score of TCM syndromes was significantly lower in the treatment group than in the control group (P<0.05), with a lower score of TCM syndromes in the treatment group than in the control group. The DHI scores were significantly decreased after treatment in the two groups (P<0.05). There was no significant difference between the two groups before treatment (P>0.05), but there was a significant difference between the two groups after treatment (P<0.05), more in the treatment group than in the control group. The average blood flow velocity of the left and right vertebral arteries and basilar artery was significantly increased in the treatment group after treatment (P<0.05), but not in the control group after treatment(P>0.05). The average blood flow velocity of the left and right vertebral arteries and basilar artery was significantly higher in the treatment group than in the control group, and the difference before and after treatment was statistically significant (P<0.05), better in the treatment group than in the control group.ConclusionsIn combination with the regular treatment in Western medicine, the modified "Old Ten Needles" therapy demonstrates remarkable therapeutic efficacy in post circulation ischemic vertigo of Qi and blood-deficiency pattern.

      摘要:ObjectiveTo evaluate the clinical efficacy and safety of acupoint acupuncture combined with acupoint application in the treatment of malignant bowel obstruction (MBO).MethodsSixty participants with MBO were randomly enrolled into the treatment group (n=30) and control group (n=30). Conventional symptomatic supportive treatment was applied to the control group while the acupuncture combined with acupoint application was given to the treatment group on the basis of symptomatic treatment. The overall clinical efficacy, Karnofsky (KPS) functional status score efficacy, obstruction symptom score, quality of life (QOL) score and safety of the 2 groups were evaluated.ResultsAs for the overall efficacy, 9 cases were completely relieved, 19 cases were improved, and 2 cases were ineffective, with a total effective rate of 93% in the treatment group. In the control group, 3 cases were completely relieved, 18 cases improved, and 9 cases were ineffective, with a total effective rate of 70%. Nonparametric test showed that as compared with the control group, acupoint acupuncture combined with acupoint application had advantages in clinical effects (Z=-2.686, P=0.007, P<0.05). Efficacy of KPS functional status scores was compared between the two groups. Among the 30 cases in the treatment group, 21 cases were improved, 5 cases were effective, 4 cases were stable, and 0 case was ineffective, with a total effective rate of 86.7%. In the control group, 13 cases were increased, 10 cases were effective, 2 cases were stable, and 5 cases were invalid, with a total effective rate of 76.7%. Nonparametric test showed that KPS functional status score efficacy was better in the treatment group than in the control group (Z=-2.106, P=0.035, P<0.05). The score of obstruction symptoms in both groups was improved after treatment, as compared with before treatment (P<0.05), and the score in the treatment group was improved significantly, as compared with the control group. The difference between the two groups before and after treatment was statistically significant (P<0.05), more in the treatment group than in the control group. In terms of QOL score, both groups were improved after treatment compared with before treatment. QOL score was more significantly improved in the treatment group than in the control group (P<0.05), indicating that the treatment group was superior to the control group. No serious adverse reactions occurred in both groups during treatment.ConclusionsThe combination of acupuncture and acupoint application shows good clinical effects in the treatment of MBO.

      摘要:ObjectiveTo analyze the medication rules of academician Wang Qi in the treatment of urticaria and provide clinical ideas for the management of this disease.MethodsAcademician Wang’s prescriptions for the treatment of urticaria were collected. The frequency of use, nature, flavor, meridian tropism and efficacy of the medicinals and association rules among medicinals in prescriptions were counted by the statistical analysis and association rule functions of the Traditional Chinese Medicine Inheritance Support Platform (V3.0).ResultsA total of 88 prescriptions were included, involving 169 traditional Chinese medicinals(TCM), and 10 TCM were used more frequently than 30 times, among which the top 5 frequently used TCM were Chandui (Periostracum Cicadae), Wumei (Fructus Mume), Lingzhi (Ganoderma lucidum), Zicao (Arnebiae Radix), and Fangfeng (Saposhnikovia Divaricate). Those medicines were mainly flat, cold and warm, the flavors were mainly sweet, pungent and bitter, the meridian tropisms were mainly attributive to the Liver Meridian, and the efficacy was mainly relieving the exterior, tonifying deficiency, clearing away heat and calming the mind. Based on association rules, 12 groups of high-frequency drug combinations were obtained, such as, "Chandui (Periostracum Cicadae), Wumei (Fructus Mume)" , "Chandui (Periostracum Cicadae), Lingzhi (Ganoderma lucidum)" , "Wumei (Fructus Mume), Fangfeng (Saposhnikovia Divaricate)" , and 10 rules of drug association were revealed, for instance, "Wumei (Fructus Mume)→Fangfeng (Saposhnikovia Divaricate)" , "Wumei (Fructus Mume), Zicao (Arnebiae Radix)→Chandui (Periostracum Cicadae)" , and "Chandui (Periostracum Cicadae), Zicao (Arnebiae Radix)→Wumei (Fructus Mume)" . ConclusionsAcademician Wang’s treatment of urticaria focuses on strengthening the essence and improving the allergic constitution, as well as cooling blood, eliminating wind and relieving itching.

      摘要:Liver cirrhosis is often derived from chronic liver diseases. In Western medicine, the treatment is centered on the treatment and prevention of cirrhosis-related complications and on liver protection. Traditional Chinese medicine has unique features as to its cognition and treatment. Liver cirrhosis, corresponds to "abdominal masses" , "swelling" and "acute jaundice" in Traditional Chinese medicine based on its staging and clinical manifestations. Professor Guan Youbo attached importance to syndrome differentiation of Qi and blood theory in its diagnosis and treatment, as diagnosis necessitates determination of etiologic factors from the perspective of qi and blood and in pattern identification. In his opinion, pathogenesis should be figured out centered on qi and blood and in treatment, and its root cause should be focused on. The root cause of liver cirrhosis is Qi deficiency and blood stagnation with dampness-heat toxins retained in the blood aspect as its symptoms. As a key part of pathogenesis of liver cirrhosis, Qi deficiency and blood stasis, which runs through the whole process of liver cirrhosis, has important effects on the formation and development of main syndrome. Phlegm and blood stasis and dampness are not only pathological products, but also the aggravation factors. In Professor Guan’s opinion, invigorating Qi and promoting blood circulation are fundamental treatments. In addition, dispelling phlegm and dampness, nourishing yin and clearing heat, promoting Qi and fluids, invigorating Qi and reviving spirits, and so on are selected according to patients’ condition. This paper introduces the application of Professor Guan’s academic thoughts of Qi and blood theory in the diagnosis and treatment of liver cirrhosis, and provides references for clinical differentiation and treatment with referrence to clinical medical records.

      摘要:The term "internal stirring of collateral wind" is a modern medical pathogenesis theory based on the rapid onset of acute cardiovascular diseases and swift changes in the condition, similar to wind in traditional Chinese medicine. Yu Genchu of the Qing Dynasty proposed that "the six meridians are the general rule for determining all diseases" . All diseases can be attributed to the six meridians. Heart chakra disease corresponds to Jueyin, in which the Jueyin pericardium instead of the heart is affected by the pathogens, and the symptoms of heart chakra disease are similar to those of Jueyin disease. Wind arises from the Jueyin, the changes in yin and yang in the Jueyin give rise to wind, and disharmony between the liver and the Jueyin stirs up wind. The "internal stirring of collateral wind" is closely related to the Jueyin diseases: Internal depression of wind and fire in the Jueyin gives rise to fire-heat, which evetually generates wind; Deficiency of the Jueyin gives rise to deficiency of the collaterals, which causes wind movement; External contraction of the Jueyin gives rise to external wind which stirs up the internal wind. The composition of Wu Mei Pills, a major formula for the treatment of Jueyin disease, contains the treatment method of dispelling wind and clearing collaterals, and also reflects the relationship between Jueyin disease and "collaterals" and "wind" . The clinical application of Wu Mei Pills in the treatment of cardiovascular diseases has also achieved good results. The dissonance between yin and yang and the disease affecting the Jueyin is an important cause of the "internal stirring of collateral wind" . It is of great significance to understand the theory of "internal stirring of collateral wind" from the perspective of six meridians.

      摘要:The obsessive-compulsive disorder is a type of mental disorder characterized by obsessive-compulsive concepts and compulsive behaviors. It is characterized by the coexistence of conscious compulsion and anti-compulsion.The clinical manifestations, occurrence and development of the obsessive-compulsive disorder are closely related to the lungs and kidneys in the five internal organs. The physiology of the lungs and kidneys is explored from the aspects of lung storage of Qi, Qi-shelving, kidneys storing essence, Jing housing the will, and from the physiological basis of the relationship between lungs and kidneys. The obsessive-compulsive disorder is located in the lungs and kidneys. Lung-kidney Qi deficiency is the cause of the disease. Lung deficiency and soul disturbance and kidney deficiency with a weak will are the core pathogenesis, and the basic treatment is to invigorate the lungs and kidneys. In terms of treatment, it focuses on invigorating the lungs and strengthening the spirit, strengthening the kidneys and strengthening the will. Under the guidance of the thoery that bitterness desires supplementation and purgation in the lungs and kidneys, and taking into account the physiological characteristics of the lungs and the kidneys, sour and bitter medicinals were used to supplemement. Combined with clinical cases, the treatment of obsessive-compulsive disorder is presented based on the theory bitterness desires supplementation and purgation in the lungs and kidneys.

      摘要:Migraine is a common neurovascular disorder. Xuanfu, also termed sweat pores, refers to microstructures as tiny as possible distributed all over the body, and its main functions include the circulation of Qi and fluids, infiltration of Qi and blood, and the operation of the Shenji. Dysfunction of the opening and closing of Xuanfu in the brain is the common foundation for the formation of various headache patterns.The important disease mechanisms of headaches include Qi depression in the Xuanfu, turbid pathogen retained in the Xuanfu, blood stasis in the Xuanfu and insufficiency of Qi and blood in the Xuanfu. Accordingly, in treatment, the basic therapeutic principle is to open the Xuanfu and regulate the blocked Qi, blood and body fluids in the human body to make it smooth. Therefore, those traditional Chinese medicinals with the effects of dispelling wind, rectifying Qi, transforming phlegm and freeing the network vessels can be chosen to treat migraine.

      摘要:The "One Qi Circumfluence" theory can be summarized as "One Qi circumfluence, the earth pivot and four images" . "One Qi" here refers to zhong Qi (middle Qi), and "four images" refer to wood, metal, fire and water organs(i.e., the liver, lungs, heart and kidneys). According to the "One Qi Circumfluence" theory, under the effects of zhong Qi, the liver on the left ascends and the lungs on the right descend, forming an "endless circulation" of the Qi activities in the human body. Vocal cord leukoplakia is a white plaque formed by keratinized hyperplasia or excessive keratinization of vocal cord mucosal epithelium. It belongs to the category of "slow throat silence" in traditional Chinese medicine. The abnormal rise and fall of Qi runs through its pathogenesis. The abnormal mediation of Qi is the source of the disease, and the dereliction of the liver and lung Qi activities is an important cause. In the treatment of vocal cord leukoplakia, it is emphasized to restore the rise and fall of Qi activities. Under the guidance of "One Qi Circumfluence" theory, restoring the zhong Qi to mediate the operation of Qi activities, regulating the rise and fall of liver and lung Qi activities and promoting the circulation of Qi of the whole body is a fundamental way to treat vocal cord leukoplakia. The "One Qi Circumfluence" theory is used to guide the prescriptions for vocal cord leukoplakia, so as to improve the clinical curative effect. It is presented in this paper with typical clinical cases.

      摘要:Arteriosclerosing occlusive disease is the most common peripheral arterial disease in clinical practice. Traditional Chinese medicine has unique advantages in treating arteriosclerosing occlusive disease. The theory of blood-vessel syndrome differentiation takes vessels as the main body and basic framework, and blood as the material basis. It holds that abnormal shape and dysfunction of vessels are two key factors in the pathogenesis of peripheral vascular diseases.The physiological characteristics of vessels are discussed by taking vessels as the physique and dredging for use, and the function of vessels is discussed by obstructing blood to communicate with ying wei and carrying Qi and blood to carry body fluids. Physiologically, blood favors warmth and dislikes cold and it serves "to help and nourish the physique and spirit" . The disease location of arteriosclerotic occlusive disease is in blood, with the blood stasis of limbs as its core pathogenesis. Based on the theory of blood-vessel syndrome differentiation, arteriosclerotic occlusive disease is divided into the patterns of yin and cold of the collaterals, blood stasis of collaterals, blood stasis with heat in the collaterals, and blood stasis and toxin accumulation in the collaterals. Combined with clinical cases, this paper introduces the treatment of arteriosclerotic occlusive disease based on syndrome differentiation.

      摘要:Hypokinetic symptoms in depression refer to symptoms such as increased fatigue, decreased activity, and obvious fatigue after exertion in patients with depression. The relationship between the liver and depression has been recognized in traditional Chinese medicine in the past, but simply soothing the liver and relieving depression can not completely improve the uncomfortable symptoms of depression patients with low motivation. The spleen is the foundation of the postnatal exsitence, the spleen governs the transportation and transformation, the spleen and the stomach are like the hub for the lifting and lowering of the Qi, and the spleen stores the intention, providing a material basis for acquired activities. Hence, regulating the spleen and stomach plays an important role in the treatment of low dynamic symptoms of depression. The pathogenesis of hypodynamic symptoms of depression is analyzed from the perspective of liver depression, which is the key pathogenesis of depression and spleen diseases. From strengthening the spleen to help the liver to rise, and waking up the spleen to help the liver to sooth the liver, the treatment of depression with low motivation from the spleen is explored. Differentiating and treating depression with hypodynamic symptoms should be combined with the method of strengthening the spleen and waking up the spleen on the basis of soothing the liver and regulating qi.

      摘要:Pancreatic cancer is a malignant tumor that occurs in the exocrine glands of the pancreas. It is difficult to diagnose early and has a high mortality rate. Traditional Chinese medicine has unique advantages in treating pancreatic cancer. The homogeny of the spleen and pancreas is recognized by most TCM physicians. Due to common symptoms of pancreatic cancer patients, it is often considered that spleen-Qi deficiency and dampness stagnation are the root causes of pancreatic cancer, and liver depression and Qi stagnation and blood stasis are factors that may contribute to the development of pancreatic cancer. In the diagnosis and treatment of pancreatic cancer, the method of invigorating the spleen and dispelling dampness are supposed to be the fundemental therepeutic method and should run from the very beginning to the end of treatment through the whole process. Combined with clinical cases, the application of invigorating the spleen and dispelling dampness in the treatment of pancreatic cancer is introduced.

      摘要:Coronary heart disease (CHD) arrhythmia belongs to the category of "palpitations" in traditional Chinese medicine. The pathogenesis includes the loss of heart support caused by deficiency of Qi, blood, yin and yang, and the pathogens disturbing the mind caused by phlegm and blood stasis. At present, there is a relative lack of therapeutic drugs for coronary heart disease(CHD) arrhythmia with the pathogenesis of pathogens disturbing the mind. For palpitations caused by phlegm and retained fluids, the method of warming and transforming phlegm should be the major one, and Linggui Zhugan Decoction is the first choice in treatment. The key point of this prescription lies in the words "Wen (warm)" and "He (and, together with)" . The word "Wen (warm)" here refers to the "warm" nature of the prescription itself, as well as the "warm temperature" of the human body. The word "He (and, together with)" includes the "sum" of time and its use in combination with other prescriptions and drugs. Combined with two cases of frequent ventricular premature beats of CHD and atrial premature beats after coronary stent implantation, this paper introduces Linggui Zhugan Decoction in the treatment of arrhythmia of CHD, so as to provide reference for clinical syndrome differentiation and treatment.

      摘要:A malignant tumor is a process of long-term, phased and multi-gene mutation accumulation. From the perspective of traditional Chinese medicine (TCM), the occurrence and development of a malignant tumor is divided into three stages: the stage of cancer toxin formation, the stage of excess pathogen accumulation and the stage of deficiency in healthy Qi and exuberance in pathogenic factors. The formation of cancer toxin is a prerequisite for the occurrence of malignant tumors. The stage of excess pathogen accumulation is the formation of precancerous lesions. The stage of deficiency in healthy Qi and exuberance in pathogenic factors is the period when precancerous lesions further deteriorate into carcinoma in situ, and develop gradually to microinvasive carcinoma, invasive carcinoma and metastatic carcinoma. Based on this, this paper puts forward the TCM prevention and treatment strategies for malignant tumors: when not ill, we should prevent the formation of pathological products such as Qi stagnation, phlegm dampness, blood stasis and cancer toxins, and establish the physical examination mode of integrated traditional Chinese and Western medicine. Attention is paid to attacking pathogens when precancerous lesions are treated, the exuberance and debilitation of the healthy qi and pathogenic factors should be distinguished when the disease is already present, and do not forget to eliminate pathogens during recovery, so as to provide ideas for TCM prevention and treatment of malignant tumors.

      摘要:Irritable bowel syndrome is the most common functional disease of the digestive system, mainly characterized by abdominal pain and accompanied by changes in bowel habits. The diarrhea-predominant irritable bowel syndrome can be treated by six meridians syndrome differentiation. The syndrome differentiation of three yang meridians mostly focuses on Taiyang Yangming patterns, Taiyang Shaoyang patterns, or Shaoyang Diseases and Yangming Meridian syndrome. Clinically, these patterns are treated with modified Gegen Decoction, Chaihu Guizhi Ganjiang Decoction, Xiao Chaihu Decoction, Gegen Qinlian Decoction, etc. Taiyin, Shaoyin and Jueyin diseases are common in the syndrome differentiation of the three yin meridians. Lizhong Decoction, Sini Powder, Wumei Pills or Xiexin Decoction are usually used clinically. Combined with clinical medical records, this paper introduces the application of six meridians syndrome differentiation in the diarrhea-predominant irritable bowel syndrome.

      摘要:IgA nephropathy is the most common primary glomerular disease. The pathogenic association between the gut microbiota and IgA nephropathy (gut-kidney axis) has attracted much medical attention in recent years. Western medicine considers the gut microbiota as a potential source of toxins in uremia, and both dysfunction of the intestinal epithelial barrier and dysbiosis of the gut microbiota accelerate the progression of IgA nephropathy. Accordingly, treatment should include regulation of the intestinal microbiota and application of new therapeutic methods. Traditional Chinese medicine believes that the attack of pathogenic qi and the damage of healthy qi are the two factors that lead to the development of IgA nephropathy, and according to the characteristics of the course of IgA nephropathy, it is divided into the acute attack phase and the chronic maintenance phase. During acute attacks, bowels are supposed to be unblocked to expel the pathogenic factors, while during chronic maintenance, the healthy Qi should be protected.
