      摘要:ObjectiveTo evaluate the clinical efficacy of Tiaogan Yangxue Jiawei Formula and acupressure ear beads in patients with breast cancer related depression.MethodsA total of 70 breast cancer patients with depression were randomly assigned to the control group and treatment group, with 35 cases in each group. After the drops-out and withdrawals excluded, there were 32 cases in the treatment group and 31 cases in the control group. The control group received conventional standardized treatment of breast cancer, and psychological counseling and health education for depression. On the basis of the control group, the treatment group received additional Tiaogan Yangxue Jiawei Formula and acupressure ear beads. Efficacy was assessed by using the Hamilton Depression Scale (HAMD), Pittsburgh Sleep Scale (PSQI), TCM syndrome score, sex hormone levels (estradiol, follicle stimulating hormone and luteinizing hormone) and safety indexes.ResultsThe HAMD score in the treatment group was lower after treatment than before treatment (P<0.05), but there was no significant before-after difference in the control group (P>0.05). Before treatment, there was no significant difference in HAMD score between the control group and treatment group (P>0.05). After treatment, the HAMD score was significantly lower in the treatment group than in the control group. The before-after treatment difference between the two groups was statistically significant (P<0.05), greater in the treatment group than in the control group.The PSQI score in the treatment group was lower than that before treatment (P<0.05), but there was no significant before-after difference in the control group (P>0.05). There was no significant difference between the control group and that before treatment (P>0.05). After treatment, the PSQI score was significantly lower in the treatment group than in the control group. There was a significant difference between the two groups before and after treatment (P<0.05), greater in the treatment group than in the control group.The score of TCM syndrome in the treatment group was lower than that before treatment (P<0.05), but there was no significant before-after difference in the control group (P>0.05). There was no significant difference between the two groups before treatment (P>0.05). After treatment, the TCM syndrome score was significantly lower in the treatment group than in the control group. The difference between the two groups before and after treatment was statistically significant (P<0.05), greater in the treatment group than in the control group.There were no cured cases in the two groups. Among the 32 cases in the treatment group, 12 cases were significantly effective, 18 cases were effective and 2 cases were ineffective. The total effective rate was 93.75%; In the control group, 3 cases were markedly effective, 15 cases were effective and 13 cases were ineffective. The total effective rate was 58.06%. By rank sum test, Z = -3.623, (P<0.05), it indicated that the curative effect was better in the treatment group than in the control group.There was no significant difference in the levels of estradiol (E2), follicle stimulating hormone (FSH) and luteinizing hormone (LH) between the two groups after treatment and before treatment (P>0.05). There was no significant difference between the two groups before and after treatment (P>0.05).ConclusionsOn the basis of modern medical routine treatment, the Tiaogan Yangxue Jiawei Formula and acupressure ear beads have a better curative effect in treating breast cancer related depression.

      摘要:ObjectiveThe clinical characteristics and medication features of patients with cerebral infarction in the real world were analyzed to provide references for clinical application.MethodsThe 6, 795 patients with cerebral infarction who were hospitalized during 2002-2015 in 37 tertially-A hospitals were included as participants, and SAS 9.3 was used to analyze frequency and percentage, and the relationship between dosage and duration.ResultsIn 6, 795 cases, the male-female ratio was 1.39∶1. People aged between 65 and 80 years old accounted for 45.95%, and workers, who were the most, accounted for 36.73%. The neurology department was the department with the most admissions, accounting for 34.64%, and the spring was the season with most admissions, acounting for 26.65%. The admission rate of Cold Dew solar term was the highest, accounting for 5.71%. The condition on admission was common, accounting for 73.33%. The hospitalization duration was mostly 15-28 days, accounting for 33.20%. Primary comobidity was hypertension, accounting for 40.71%. Among all traditional Chinese medicine patterns, the pattern of phlegm and blood stasis blocking collaterals was the most common, accounting for 18.16%.The administration of breviscapin for injection was the most frequently used intravenous drip, totaling 9, 800 times, accounting for 89.01%, of which 47.67% were not and 52.32% had off-label doses.The average medication days were 11.12 ± 9.63 d. The treatment of cerebral infarction mainly included antihypertensive drugs, antiplatelet aggregation drugs, neuroprotection drugs, and improvement of cerebral blood supply drugs. The treatment of cerebral infarction with the combination of related traditional Chinese medicine mainly included promoting blood circulation and removing blood stasis, nourishing Qi and Yin, resusinating, nourishing Qi and strengthening Qi, pacifying and tonifying the liver and kidney, resolving phlegm and lowering lipid. Among the cured patients, 44.84% of users were within use-standard dosage, and 55.16% were over-standard dosage. Among patients with improved conditions, 52.91% were within use-standard dosage, and 47.09% were over-standard dosage.ConclusionsIn the real world, breviscapin for injection is often used for the treatment of cerebral infarction, which is mostly combined with Western medicine of hypotensor and traditional Chinese medicine of promoting blood circulation and removing blood stasis. Further research is needed to study the efficacy of breviscapine for injection and its safety when combined with glucocorticoids and medicine of promoting blood circulation and removing blood stasis.

      摘要:ObjectiveTo observe the clinical efficacy of Yiqi Huoxue Formula combined with heparin sodium injection in the treatment of the acute deep venous thrombosis in the lower limbs.MethodsSeventy patients with acute lower extremity deep venous thrombosis and with the Qi deficiency and blood stasis pattern were randomly assigned to the control group (n=35) and treatment group (n=35). After the drops-out and withdrawals excluded, there were 33 cases in the control group and 33 cases in the treatment group. The control group was treated with heparin sodium injection, and the treatment group received additional Yiqi Huoxue Formula on the basis of the treatment of the control group. Clinical efficacy, skin temperature, skin color, pain, swelling of affected limbs, Homans sign, ultrasonic changes and other quantitative scores of affected limbs, coagulation indexes such as D-Dimer (D-D), fibrinogen (FIB) and antithrombin Ⅲ (AT-Ⅲ), high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α)and other inflammatory indexes were observed, and the safety was assessed.ResultsIn the treatment group, 10 cases were cured, 11 cases were markedly effective, 10 cases were effective and 2 cases were ineffective. The total effective rate was 93.94%; In the control group, 7 cases were cured, 7 cases were markedly effective, 9 cases were effective and 10 cases were ineffective. The total effective rate was 69.70%. Nonparametric test showed that the clinical effect was better in the treatment group than in the control group (Z=-2.150, P=0.032, P<0.05). After treatment, the skin temperature, skin color, pain, swelling of affected limbs, Homans sign and ultrasonic change scores in the two groups were lower than those before treatment (P<0.05). After treatment, the skin temperature, skin color, pain, swelling of affected limbs, Homans sign and ultrasonic change scores in the treatment group were significantly lower than those in the control group (P<0.05). After treatment, D-D and FIB in the two groups were lower than those before treatment, and AT-Ⅲ was higher than those before treatment (P<0.05). After treatment, D-D and FIB were significantly lower in the treatment group than in the control group, and AT-Ⅲ was significantly higher in the treatment group than in the control group (P<0.05), better in the treatment group than in the control group. After treatment, the hs-CRP, IL-6, TNF-α in the two groups were lower than those before treatment (P<0.05). After treatment, hs-CRP, IL-6, TNF-α were significantly lower in the treatment group than in the control group (P<0.05), better in the treatment group than in the control group.ConclusionsYiqi Huoxue Formula combined with heparin sodium injection has a good clinical effect in the treatment of acute deep venous thrombosis of the lower extremities.

      摘要:ObjectiveTo observe the clinical efficacy of nephrotic formulas by Professor Peng Jianzhong for chronic kidney disease. MethodsA total of 374 patients with chronic kidney disease were selected and treated with nephropathy formulas, without limiting the basic treatment of Western medicine, including diet, blood glucose and blood pressure. The changes in creatinine (Cr), uric acid (UA), urea nitrogen (BUN), glomerular filtration rate (GFR), triglyceride (TG), total cholesterol (TC), 24-hour urine total protein quantification, urinary protein and urinary occult blood were observed before and after treatment.ResultsAfter treatment, Cr and UA decreased (P< 0.05) and GFR increased (P<0.05), and there was no significant difference before and after BUN treatment (P> 0.05). There was no significant difference in TG and TC before and after treatment (P> 0.05). The 24-hour urine total protein quantification was lower after treatment than before treatment (P<0.05). The urinary protein after treatment was better than that before treatment (P<0.05), and the urinary occult blood after treatment was better than that before treatment (P <0.05).ConclusionsThe nephrotic formulas that Professor Peng Jianzhong uses to treat chronic kidney disease have a good clinical effect.

      摘要:ObjectiveTo explore the medication rules of Chinese patent medicine for chronic bronchitis, in the hope of providing referrence for its clinical treatment and new drug development.MethodsChinese patent medicines for chronic bronchitis prescriptions were collected from Chinese Pharmacopoeia. The Traditional Chinese Medicine Inheritance Support System was used to count indications and frequency of traditional Chinese medicine. The rules of TCM compound and association were analyzed by using the method of association rules. Core medicinal combinations and candidate new prescriptions were extracted based on entropy clustering.ResultsEighty kinds of Chinese patent medicines for chronic bronchitis treatment were found in Chinese Pharmacopoeia. Among them, 14 kinds of Chinese patent medicines had more than 2 dosage forms, totaling 102 kinds, and the dosage form was mainly pills which are 27 kinds. The main functions of Chinese patent medicines of chronic bronchitis are relieving cough, transforming phlgem, relieving panting, and also clearing heat, clearing the lung, dispersing the lung, moistening the lung and downbearing qi.A total of 80 kinds of Chinese patent medicines for the treatment of chronic bronchitis were included, involving 167 medicinals, and the frequency of medicinals reached 882 times, 24 medicinals with its frequency ≥ 7. Most frequently used medicinals were Gancao (Radix et Rhizoma Glycyrrhizae), Kuxingren (Semen Armeniacae Amarum), Jiegeng (Radix Platycodonis), Mahuang (Herba Ephedrae) and Chenpi (Pericarpium Citri Reticulatae). There were 26 medicinal combinations, with its frequency≥10, for instance, Gancao and Kuxingren, Gancao and Jiegeng, Chenpi and Gancao, Mahuang and Kuxingren. Twenty-four medicinals combinations were revealed, with its frequency≥10, for instance, "Jiegeng, Kuxingren→Gancao" "Chenpi, Jiegeng→Gancao" . Based on complex system entropy clustering, a group of 8 core drug combinations were obtained. Based on unsupervised entropy hierarchical clustering, 4 new prescriptions for treatment of chronic bronchitis were obtained.ConclusionsThe common treatment for chronic bronchitis is to treat both the symptoms and the root causes. Treating the symptoms include relieving cough, transforming phlegm and relieving panting while treating the root causes include coordinating viscera and bowels and regulating Qi. The TCM treatment of chronic bronchitis is flexible, based on clinical syndrome differentiation.

      摘要:Rheumatism refers to a group of heterogeneous diseases characterized by invaded joints, bones, blood vessels, muscles and surrounding soft tissues (bursa, tendons, fascia, etc.) or connective tissue, most of which are autoimmune diseases. Dihuang Yinzi has the functions of tonifying both Yin and Yang, enriching Yin and moistening Yang, supporting water and fire, cultivating the foundation and strengthening the yuan. Professor Fang Dingya advocates integrating disease differentiation and syndrome differentiation. He is good at treating specific diseases with specific prescriptions and drugs. In his opinion, Dihuang Yinzi can improve physical constitution, regulate hormone levels and immunologic function. It can treat degenerative diseases caused by lack of vitality and kidney essence, especially for weak patients due to chronic rheumatic disease. Combined with three cases of rheumatoid arthritis complicated with Sjogren’s syndrome, polymyositis and cervical spondylotic myelopathy, Professor Fang’s clinical experience in using Dihuang Yinzi to treat rheumatic diseases is presented in this article.

      摘要:Chronic prostatitis refers to the disease characterized by pelvic pain or discomfort and abnormal urination under the action of pathogens or some non-infectious factors. It belongs to the category of "bai yin" and "essence turbidity" in traditional Chinese medicine. Professor Li Yueqing believes that the basic pathogenesis of chronic prostatitis is kidney deficiency, dampness-heat and blood stasis, in which kidney deficiency serves as the foundation, dampness-heat as its manifestation and blood stasis as the change. Professor Li advocates that in the treatment of chronic prostatitis, the combination of warming method and clearing method should be used to restore the function of the body and clear away the pathogenic factors in a broad sense. In clinical practice, the combination of warming and clearing is often used, which changes flexibly and increases and decreases with the symptoms, and it has achieved good clinical curative effects. Combined with clinical medical records, this paper introduces Professor Li’s diagnosis and treatment ideas and medication experience in the treatment of chronic prostatitis with warming method and clearing method.

      摘要:Peripheral vascular diseases mainly include arterial, venous, and lymphatic diseases. Most peripheral vascular diseases have such symptoms as cold limbs, pain, intermittent claudication, swelling of limbs, and numbness of the skin. It may develop into ulcers or gangrene in severe cases. Traditional Chinese medicine believes that the pathogenesis of these diseases is mostly yang deficiency, cold coagulation, and blood stasis. Professor Pang He believes that peripheral vascular diseases correspond to the "blood paralysis" in the Synopsis of the Golden Chamber, and advocates treating peripheral vascular diseases with Huangqi Guizhi Five-Ingredient Decoction, which is indicated for blood paralysis. Professor Pang’s experience in peripheral vascular treatment with modified Huangqi Guizhi Five-Ingredient Decoction is introduced from several aspects, for example, treating arteriosclerosis obliterans in accordance with disease severity, curing chronic lower extremity venous edema by combination of paired or triangular Chinese medicine, treating Raynaud’s syndrome by grasping syndrome characteristics, and curing lower extremities by coupling the method of "adding water to help the boat move" and insect medicine. Besides, the treatment ideas and medication experiences of Professor Pang are illustrated based on his clinical treatment cases.

      摘要:Allergic rhinitis (AR) is a noninfectious chronic inflammatory disease of nasal mucosa mainly mediated by immunoglobulin E exposed to allergens. Its typical clinical features are as follows: paroxysmal sneezing, stuffy nose, rhinocnesmus and nasal obstruction. It is named as Biqiu in traditional Chinese medicine and it falls into the category of otorhinolaryngology. The pathogenic characteristics of AR in autumn were discussed from the aspects of natural environmental factors, living environmental factors, seasonal factors, exogenous pathogenic factors and latent pathogenic factors. Combined with these discussions, it attempts to provide clinical references for prevention and treatment for AR based on syndrome differentiation.

      摘要:The tic disorders are neuropsychiatric disorders characterised by involuntary, repetitive, sudden, rapid, recurrent and nonrhythmic motor tics and/or phonic tics in one or more parts of the body, and they often occur in children. In traditional Chinese medicine, the tic disorders in children are classified as liver wind, located in the liver and closely related to the spleen. The basic pathogenesis is the liver wind stirring, the wind phlegm obstructing collaterals is the reason why this disease is difficult to cure, and the liver depression and spleen obstruction are the resason for its progression. Triangular herbs are a combination of three herbs, which are formulated according to the property, flavor, and meridian of the herb. Triangular herbs are widely used in the treatment of the tic disorders in children. According to the pathogenesis, three methods are formulated in treating children with the tic disorders, namely, regulating the liver and calming wind to stop tics, calming wind and expelling phlegm to dredge meridians and transporting the spleen and soothing the liver to regulate Qi. Therefore the fixed triangular herbs are formed accordingly. In terms of treatment, triangular herbs are used flexibly to form a traditional Chinese medicine prescription based on the holistic dialectics and the combination of disease differentiation and syndrome differentiation. Combined with clinical case, this paper introduces the clinical application of triangular herbs in children with the tic disorders.

      摘要:Ulcerative colitis is a chronic nonspecific inflammatory bowel disease characterized by continuous and diffuse inflammatory changes in the colorectal mucosa. Its clinical symptoms include abdominal pain, diarrhea, bloody purulent stools, tenesmus and so on. It can be classified into the categories of "intestinal diarrhea" , "dysenteric diarrhea" and "rest dysentery" in traditional Chinese medicine. In traditional Chinese medicine, the basic pathogenesis of the disease is deficiency of healthy Qi, retention of dampness and toxin and stagnation of Qi and blood. The disease is located in the large intestine and is closely related to the liver, spleen and kidney.Wind medicinals are pungent in flavor, thin and light in texture, and has the nature of rising and floating. According to the property and function of wind medicinals, combined with the physiological and pathological characteristics of the spleen and stomach, this paper discusses the mechanism and application of wind medicinals in the treatment of ulcerative colitis from the aspects of lifting spleen Yang, dispelling wind and eliminating dampness, promoting stagnation, soothing the liver and regulating Qi, promoting blood circulation and removing stasis, and guiding the meridian. Combined with a clinical case, this paper introduces the application of wind medicinals in the treatment of ulcerative colitis.

      摘要:Childhood obesity is primarily simple obesity, and Chinese medicine treatment has obvious advantages for it. The statement of "Yang transforming Qi, Yin forming" , from Basic Questions·(Suwen·Yin-Yang Yingxiang), expounds the diagnosis and treatment of simple obesity in children from the perspective of "yang transforming Qi and Yin forming" . The imbalance between "Yang transforming Qi and Yin forming" is the key pathogenesis of obesity. According to the syndrome differentiation of "Yang transforming Qi, Yin forming" , childhood obesity at various stages is analyzed, and the method of supporting yang and transforming yin is used to guide the prevention and treatment of simple obesity in children, including life care, traditional Chinese medicine treatment, comprehensive external treatment, etc. At the same time, combined with clinical medical records, the diagnosis and treatment experience of treating simple obesity in children from "Yang transforming Qi, Yin forming" is introduced.

      摘要:Breast cancer is one of the most common malignant tumors in women. Combined treatment of traditional Chinese medicine based on Western medicine can greatly improve its prognosis. Deficiency of Yin and Yang constitutes the primary pathogenesis of breast cancer, and hence its treatment can be performed from the aspects of Yang deficiency syndrome and Yin deficiency syndrome. For patients with the Yang-deficiency breast cancer, there is a high risk of affection by exogenous pathogenic factors of wind, cold and dampness; and patients may lose the abilities of Qi-promotion and Qi-warming, leading to phlegm dampness blood stasis with time eventually. These patients may manifest the syndrome characteristics of scattered swelling, unchanged skin color, no pain or itching and systemic Yang deficiency in the early stage of the lesion. Its syndrome shares similarities with "carbuncles of Yin nature" , and it can be treated with Yanghe Decoction. Meanwhile, for patients with Yin-deficiency breast cancer, there is a risk of pathogenic factors invading the hepatic collaterals, which may remain in the Ying-blood and cause heat transmission over time. These patients may manifest the syndrome characteristics of high risk of local ulceration, metastasis, progression to late stage and systemic Yin deficiency. Its syndrome is similar to a "pathogen lurking in the Yin phase" , and it can be treated with Qinghao Biejia Decoction. Significantly, under the guidance of the theory of reinforcing Yang from Yin and reinforcing Yin from Yang and considering the disease characteristics of systemic deficiency of healthy Qi and local excess of pathogenic factors, Yanghe Decoction and Qinghao Biejia Decoction can also be applied jointly, associated with external treatment.

      摘要:The cultivation of postgraduates with master’s degree in tranditional Chinese medicine is aimed at clinical high-level talents of traditional Chinese medicine, and its training quality is related to the development of traditional Chinese medicine and people’s health level. Since 2012, the First Clinical Medical College of Beijing University of Chinese Medicine has deeply promoted the reform of the joint cultivation of postgraduates with master’s degree in traditional Chinese medicine and the standardized training of Chinese medicine residents. This paper introduces the background and basis of the reform and implementation of the training mode of postgraduates majoring in master of traditional Chinese medicine from the aspects of the differences between the objective of degree education and vocational education, the integration of professional education and moral education, the improvement of clinical practice teaching system, the optimization of professional degree curriculum system, and the reconstruction of assessment and quality monitoring mechanism. This paper explores the reform and practice of the cultivation mode of postgraduates of master’s degree of Chinese medicine from the aspects of constructing the "cooperation between medicine and education and integration of general and specialized courses" , establishing the subjective moral education system of "Building Morality and cultivating people and four-in-one" , constructing the clinical practice teaching system of "through training and combination of deficiency and reality" , creating the "3 + X" inheritance and innovation curriculum system, and reconstructing the assessment and evaluation mechanism and quality monitoring mechanism. The practice of talent training in the past decade has proved that the reform of the training mode of postgraduates with master’s degree in Chinese medicine has achieved good results in talent training, student benefits, employment quality, industry demonstration, social impact and so on.

      摘要:Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) is a chronic inflammatory disease occurring in the cerebral bridge, midbrain and perivascular cerebellum, characterized by lymphocytic infiltration, and treated effectively with the steroid hormone therapy, which is rare clinically and treated mainly with the hormone shock therapy at home and abroad. We used the method of tonifying the heart and tranquilizing the mind, nourishing the blood, transforming phlegm and unblocking collaterals, and used the heart-regulating and mind-tranquilizing acupuncture method combined with the hormone therapy to treat one case with CLIPPERS, which achieved satisfactory results and provided ideas for the treatment of this disease in traditional Chinese medicine.
