      摘要:ObjectiveTo explore the effect of acupuncture at Shenmen(HT 7)on the activity of amplitude of low frequency fluctuation (ALFF) after 24 h sleep deprivation based on cortical analysis.MethodsA total of 30 healthy subjects were recruited, and received the needling at bilateral Shenmen acupoints (HT 7) after 24 h sleep deprivation. The functional magnetic resonance scanning (fMRI) was performed before and after sleep deprivation, and after acupuncture at HT 7. Meanwhile, DPABISurf was used to preprocess and calculate the ALFF values.ResultsCompared with that before sleep deprivation, the brain regions with increased ALFF values after sleep deprivation mainly included bilateral posterior central gyrus, bilateral anterior central gyrus and right paracentral lobule, meanwhile the brain regions with decreased ALFF values mainly include bilateral angular gyrus, bilateral posterior cingulate gyrus and right inferior frontal gyrus. Compared with that after sleep deprivation, the brain regions with increased ALFF values after needling at HT 7 mainly included the right superior orbitofrontal gyrus and inferior frontal gyrus, and the brain regions with decreased ALFF values included the bilateral posterior central gyrus, left anterior central gyrus, bilateral paracentral lobules, and left insula. Compared with that before sleep deprivation, there was no significant change in ALFF value in brain regions after acupuncture.Conclusions Acupuncture at HT 7 can modulate the abnormal functional activities of multiple brain regions related to cognitive functions and emotional regulation after sleep deprivation, which may be the effect mechanism of acupuncture on sleep deprivation.

      摘要:ObjectiveTo investigate the clinical effect of acupuncture and cupping therapy on acute tonsillitis in children.MethodsA total of 102 children with acute tonsillitis who were admitted to the Third Affiliated Hospital of Beijing University of Chinese Medicine from March 2019 to September 2020 were recruited as the subjects. A prospective, parallel open control study was conducted and they were randomly divided into the control group and the observation group (n=51). The control group was only given symptomatic treatment, while the observation group was treated with acupuncture and cupping on Dazhui (DU 14) and acupuncture and bloodletting on Shaoshang (LU 11) on both hands on the basis of symptomatic treatment. If the subjects in the observation group still reported fever 2 days after treatment, the above-mentioned TCM treatment was given again. The time needed for the fever to go away, for pharyngeal pain to improve, for pharyngeal congestion to improve, for tonsillar enlargement to improve and for tonsillar abscess to disappear was recorded. Syndrome scores were obtained and the effective rates of the two groups were collected. WBC, N% and CRP values before and 5 days after treatment were acquired for comparison. A two-week follow-up was conducted to obtain information about disease recurrence.ResultsThe total effective rate of the observation group was 98.04%, higher than that of the control group (90.20%), but the difference was not statistically significant (P>0.05). The time of fever reduction, improvement of pharyngeal pain and improvement of pharyngeal congestion in the observation group were shorter than those in the control group, and the differences were statistically significant (P<0.05). Compared with that of the control group, the improvement time of tonsil enlargement and the disappearance time of tonsil abscess in the observation group were also shortened, but the difference was not statistically significant (P > 0.05). After treatment, WBC, N% and CRP in the two groups were lower than those before treatment (P < 0.05), and CRP in the observation group was significantly lower than that in the control group (P < 0.05). It was found during the follow-up that, 2 weeks after treatment, the recurrence rate of observation group was significantly lower than that of control group (P < 0.05).ConclusionCompared with conventional therapy, acupuncture and cupping therapy is effective in improving clinical symptoms and signs, shortening the course of disease, alleviating acute inflammatory response and preventing recurrence.

      摘要:ObjectiveTo observe the efficacy and safety of endocrine-regulating acupuncture in the treatment of ovulation disorders in polycystic ovary syndrome.MethodsFrom June 2020 to February 2021, 65 patients were recruited who met the inclusion criteria in the Acupuncture Clinics and Gynecological Clinics of Guangdong Provincial Hospital of Chinese Medicine. The simple randomization method was used and they were randomly divided into two groups according to the order of their inclusion, namely the control group (clomiphene group, 33 cases) and the treatment group (endocrine-regulating acupuncture group, 32 cases). Both groups were given lifestyle guidance. The control group was given clomiphene citrate tablets, starting from the 2nd to 5th day of menstruation, 50 mg/d, for 5 days; the treatment group was given endocrine-regulating acupuncture, once every other day, three times a week. The two groups were treated for 3 menstrual cycles, and the changes in sex hormone levels, TCM symptom scores, ovulation and pregnancy were observed and the differences were identified.Results① Ovulation rate: The ovulation rate of the treatment group was 60.87%; the control group was 63.24%; the difference was not significant after statistical analysis (P > 0.05). ② Sex hormone level: after treatment, follicle-stimulating hormone (FSH) in the two groups was significantly higher than before treatment; luteinizing hormone (LH), LH/FSH, testosterone (T) were significantly lower than before treatment, with statistical significance (P < 0.05).The reduction of LH, LH/FSH and T in the treatment group was greater, and the difference was significant (P < 0.05); in terms of FSH increase, the difference between the two groups was not statistically significant (P > 0.05). ③ Pregnancy rate: The pregnancy rate of the treatment group was 42.86%; the pregnancy rate of the control group was 41.94%. After comparison, there was no significant difference between the two groups (P > 0.05).ConclusionEndocrine-regulating acupuncture can improve ovulation disorders in polycystic ovary syndrome, help promote ovulation and fertility, and improve sex hormone levels.

      摘要:The "yangming channels pathogenic theory (èr yáng zhì bìng)" is an ingenious academic viewpoint created by Professor Chai Songyan, laureate of the title "National Master of Traditional Chinese Medicine" , who has been engaged in clinical diagnosis and treatment of gynecological diseases for more than 60 years. It is of great significance for the treatment of amenorrhea caused by antipsychotic drugs. This paper, by summarizing Prof. Chai’s academic thoughts, aims to provide a new perspective for the TCM treatment of amenorrhea caused by antipsychotics.

      摘要:Severe upper abdominal pain, as one of the diagnosis criterions of acute pancreatitis (AP), is closely related to the clinical outcomes and subjective feelings of patients. Elevated evidence of clinical and basic studies indicated that pain, as an early event, may relate to the development and severity of AP. Pancreatic nerve transmits sensory information and enhances the synthesis and release of local inflammatory cytokines, a process called neurogenic inflammation, which may aggravate the damage to the pancreas. This process is coincided with the pathogenesis of "pathogen constraint-excessive heat-qi block" in AP and the "obstruction causes pain" theory in TCM. The authors illustrated the role of neural mechanism in AP, also the TCM pathogenesis of the disease and related clinical evidence.

      摘要:Tic disorders are often caused by the onset, aggravation or even recurrence of allergic rhinitis. It is important that good treatment of tic disorders with allergic rhinitis can prevent the disease, reduce recurrence and shorten the course. This paper discusses the relationship between spirit and qi. The conclusion is that qi movement, which consist of ascending, descending, exiting and entering, is the foundation of children’s growth both in body and mind. On this basis, it further puts forward the pathogenesis of tic disorders with allergic rhinitis is closely related to the external wind driving the internal wind. The location of this disease is the liver and lung. Furthermore, the core pathogenesis of it is the imbalance of qi movement. Therefore, the therapeutic principle is to restore the smooth and balanced movement of qi, regulate the lung and calm the liver, with necessary additions to the formula based on the symptoms. In the acute stage, it is emphasized that the symptoms should be solved first, and then the root causes later. During the remission period, emphasis is placed on the root, while dealing with the branches at the same time. From the perspective of qi movement in children, this paper provides ideas about syndrome differentiation and treatment of tic disorders with allergic rhinitis, to hopefully serve as guidance for the TCM treatment of the disease that focuses on "preventing disease before it arises and controlling the development of existing disease" .

      摘要:Stagnation syndrome (yù zhèn), or constraint syndrome, is a general term for the stagnation of things that are difficult to be dissipated, which is manifested in a fashion similar to depression in western medicine. The concept of mèi bìn in TCM refers specifically to sleep disorders and is also one of the manifestations of stagnation syndrome. It is common in clinical practices that stagnation syndrome is closely related to insomnia, and may also lead to the other extreme: somnolence. The pathogenesis of insomnia and somnolence mainly involves the liver and heart, and is closely related to phlegm and blood stasis. Ancient and modern literature and prescriptions have confirmed that the connection between stagnation syndrome and the two sleep-related diseases is remarkably close and commonly seen. In modern medical research, depression has been proven to cause insomnia and somnolence. There are also records of mental and psychological factors that are in play as well as their mechanisms and antidepressant therapies.

      摘要:The theory of "yang transforms into qi while yin constitutes form" is the general outline of the yin-yang theory in traditional Chinese medicine, which embodies the concept of life contained in the ancient philosophical thought of China’s ancient medical practice. The spermatozoa can move and fertilize under the warming and propulsive effect of "yang transforms into qi" , while they are produced and their morphological and structural integrity is maintained under the cohesive effect of "yin constitutes form" . The malfunctioning of "yang transforms into qi while yin constitutes form" is an important pathogenesis of male infertility. Abnormality in the process of "yang transforms into qi" leads to insufficient activity and motility rate of the sperm, and dysfunction of "yin constitutes form" may induce low sperm count and high sperm deformity rate, and even high sperm DNA fragmentation rate. This theory can thus provide some insights into male infertility. TCM syndrome differentiation and treatment of the disease should be based on its pathogenesis, focusing on warming and activating kidney yang and aiding the transforming of yang into qi, enriching and nourishing kidney yin and condensing yin, and harmonizing yin and yang, tonifying the kidney and supplementing the essence. The clinical medication should follow the characteristics of using more herbal medicine of the seed category and adding animal-based ingredients when needed, and the creation of the prescription should be guided by the theory of "a stem formula for the dominant pathomechanism" . Jujing Decoction (Essence-Gathering Decoction) nourishes kidney yin while warming kidney yang to seek yin within yang, and also invigorates blood and dissolves stasis, with the recipe containing primarily seed medicine, and also blood and flesh products when appropriate. It has been shown in a previous study that Jujing Decoction can effectively improve sperm vitality, reduce sperm malformation rate and improve sperm DNA fragmentation index, and has a desirable safety profile.

      摘要:Diabetic peripheral neuropathic pain (DPNP) is one of the common complications of diabetes. At present, the conventional treatment for DPNP still focuses on relieving pain and depression. Traditional Chinese medicine has some advantages in treating this disease. Combining the clinical characteristics of DPNP, this study adopts the six meridians dialectical theory to explore the progression and pattern characteristics of DPNP, and divides it into four types: evil entering the taiyin meridians and remaining in the yang meridians, evil entering the taiyin and jueyin meridians, evil entering the taiyin and shaoyin meridians, and evil entering the shaoyin and jueyin meridians. Corresponding treatment method and the concept of prevention before disease are also put forward, so as to provide a new perspective for the treatment of diabetic peripheral neuropathic pain.

      摘要:ObjectiveTo study the effect of 60Co γ-ray on secretion of inflammatory factors by Sertoli cells, and to explore the protective mechanism of Yiqi Jiedu Decoction (Qi-Boosting Toxin-Resolving Decoction) on the secretory function of Sertoli cells injured by the radiation.MethodsSertoli cells were divided into Normal Control (NC) group, Ionizing Radiation (IR) group, Yiqi Jiedu Decoction high dosage (YQJD-H) group, Yiqi Jiedu Decoction middle dosage (YQJD) group, Yiqi Jiedu Decoction low dosage (YQJD-L) group, NF-κB inhibitor (Bay) group and Yiqi Jiedu + NF-κB inhibitor (Y+ B) group. The groups were cultured under corresponding conditions for 24 hours. Then, radiation-damaged cell models were created by putting all groups except the NC group under 60Co γ-ray radiation at 12 Gy. The CCK8 method was used to detect the cell viability of the Sertoli cells, Elisa to detect the content of IL-1β, IL-6, TNF-α, TGF-β in cell supernatant, and Western Blot to detect the protein expression level of the NF-κB/IκB α pathway.Results24 hours after irradiation, compared with the NC group, the proliferation of Sertoli cells in the IR group decreased (P < 0.01). The secretion of IL-1β, IL-6, TNF-α, TGF-β increased (P < 0.01). The protein expression level of NF-κB and phosphorylated NF-κB increased (P < 0.01). The protein expression level of IκB α decreased (P < 0.01). A comparison with the IR group reveals that Yiqi Jiedu decoction can increase the cell viability of Sertoli cells (P < 0.01). It can also significantly reduce the content of IL-1β, IL-6, TNF-α, TGF-β (P < 0.01, P < 0.01, P < 0.05, P < 0.01). The protein expression level of NF-κBin in the YQJD group, Bay group and Y+ B group decreased (P < 0.05, P < 0.001, P < 0.01). The protein expression level of phosphorylated NF-κB in the YQJD group and Y+ B group decreased (P < 0.01). The protein expression level of IκB α in the YQJD group, Bay group and Y+ B group increased.Conclusion60Co γ-ray radiation at 12 Gy reduced the proliferation ability of Sertoli cells and disordered their secretory function. Yiqi Jiedu Decoction can significantly reduce the radiation damage to Sertoli cells by regulating NF-κB/IκB α pathway, and promote the restoration and stability of testicular microenvironment to a certain extent.

      摘要:ObjectiveTo systematically evaluate the methodological quality of the guidelines for the diagnosis and treatment of chronic kidney diseases in traditional Chinese medicine/integrated traditional Chinese and Western medicine, so as to provide a basis for further update and promotion of the guidelines.MethodsSystematical search was performed of the traditional Chinese medicine/integrated Chinese and Western medicine guidelines for chronic kidney diseases diagnosis and treatment published by Chinese journals. The information about their characteristics were extracted and summarized. Then AGREE Ⅱ was used in independent quality evaluations in six aspects including clarity of expression, scope and purpose, participants, rigor, editorial independence, and applicability. The scoring results are analyzed in layers based on the year of their publication, institution, development method, and whether they are funded or not, and are horizontally compared with the published AGREE Ⅱ evaluation results of guidelines for other diseases at home and abroad.ResultsA total of 19 guidelines were included, of which: 2 guidelines for traditional Chinese medicine, 17 guidelines for integrated traditional Chinese and western medicine, covering diseases such as chronic renal failure and IgA nephropathy; 16 were formulated by academic societies, and 1 was an updated and revised version, 4 were formulated using evidence-based medicine methods, and 8 guidelines received research funding; the guidelines were issued from 2008 to 2020, and 3 were formulated within the past 5 years. In the overall AGREE Ⅱ score, the clarity of expression (52.63%), scope and purpose (42.4%) scored higher, followed by the score of participants (24.27%). Rigor (19.19%), editorial independence (18.42%), and applicability (7.24%) scores were very low, and 5 guidelines (26.31%) were "recommended for use" by the evaluators; In the stratified analysis, it was found that the AGREE Ⅱ scores of the three types of guides were relatively high: recently developed guidelines, those which used evidence-based medicine method and those funded by scientific research funds; The AGREE II scores of TCM/Integrated medicine guidelines in the field of chronic kidney diseases in China and in the cardiovascular field are equivalent, but the results are quite different with the AGREE II evaluation of international guidelines.ConclusionThe AGREE Ⅱ score of the guidelines for the diagnosis and treatment of chronic kidney diseases in traditional Chinese medicine/integrated traditional Chinese and Western medicine is generally low, and more attention should be paid to evidence-based methodological principles, especially in terms of the staff, the rigor of technical processes, conflicts of interest, and applicability.

      摘要:ObjectiveTo analyze clinical experience in treating acne vulgaris based on data mining.MethodsThe frequency, classification, efficacy, the four qi, the five flavors and channel tropism of TCM herbal medicine in them were statistically analyzed. Association rules and clustering analysis were used to explore potential herbal ingredient combinations, and data visualization was realized by network diagrams.ResultsA total of 395 prescriptions were included in the study, involving 212 ingredients. The most frequently used ones include Spina Gleditsiae, Radix et Rhizoma Glycyrrhizae, Fructus Forsythiae, Radix Scutellariae, and Radix Angelicae Dahuricae. The herbs are mainly cold and warm medicinals in terms of medicinal properties, and bitter, acrid, and sweet in terms of flavors. The meridian that they enter is primarily the spleen meridian. The most frequently seen category is the heat-clearing medicinals, followed by the deficiency-supplementing medicinals, the blood-invigorating and stasis-dissolving medicinals. The mostly seen function of the herbs is tonifying qi, followed by clearing heat and resolving toxins, draining water and reducing swelling, and clearing heat and drying dampness. The herb with a eigenvector centrality of 1 in the complex network graph is Spina Gleditsiae, followed by Radix Angelicae Dahuricae, Fructus Forsythiae, Radix Scutellariae, and Radix et Rhizoma Glycyrrhizae. Association rule analysis yields 34 combinations of TCM herbal medicine commonly used for acne. In the cluster analysis, 3 candidate new prescriptions are obtained.ConclusionOur team attaches importance to the application of heat-clearing medicine, blood cooling and invigorating medicine when treating acne vulgaris, and also focuses on regulating the spleen and stomach.

      摘要:Professor Sun Pei is the fourth-generation heir of Sun’s School of Traditional Chinese Medicine (including the technique of percussopunctator), and a fourth-batch laureate of the title of Prestigious TCM Doctor in Beijing. Over decades of clinical practice, he has always paid attention to the pathogenic theory of internal and external toxins and the pathogenesis transformation of yin-yang balance in yang and yin-yang balance in yin. On the basis of identifying diseases, he has attached great importance to distinguishing between symptoms, different treatment methods for the same disease and one protocol for different diseases. In terms of the treatment of insomnia, the therapeutic idea of harmonizing yin and yang as the core and adjusting zang-fu organs and qi-blood as the auxiliary has been formed. His ingenious insights obtained from clinical experience into the formulation of prescriptions emphasize the combined use of Rhizoma Pinelliae and Spica Prunellae, as well as Periostracum Cicadae and Caulis Polygoni Multiflori to harmonize yin and yang. For various complications of insomnia, the formula is adjusted based on syndrome differentiation: for patients with stagnation disease, Receptaculum Nelumbinis, Bulbus Lilii, Radix et Rhizoma Gentianae, Radix Linderae should be added; for those with blood stasis, Radix et Rhizoma Salviae Miltiorrhizae and Rhizoma Chuanxiong are added; if severe fire constraint is present, the physician should add Flos Chrysanthemi, Fructus Gardeniae Praeparatus and Semen Sojae Praeparatum; for the loss of nourishment of the heart spirit, Fructus Tritici Levis, Radix et Rhizoma Glycyrrhizae Praeparata cum Melle, Fructus Jujubae, Sclerotium Poriae Pararadicis and Radix et Rhizoma Cynanchi Paniculati are added; for obstruction of both phlegm and qi, the formula should be enhanced with Rhizoma Acori Tatarinowii, Radix Polygalae and Pinellia and Officinal Magnolia Bark Decoction; Dens Draconis, Concha Ostreae, Concha Margaritiferae Usta are proper additions to the prescription for restlessness of the heart spirit and internal stirring of liver wind.
