摘要:Prof. Wang Qingguo put forward the academic idea of "achieving harmony by unblocking and balancing" . He believes that "to determine the therapeutic method according to the pattern of the patient" is actually to base the treatment on the pathomechanism, and that the notion of "the prescription stemming from the therapeutic method " means creating the formula on the basis of the treatment method obtained with the pathomechanism. He advocates that disease mechanism and qi movement are two manifestations of the same thing, and qi is the link between pathomechanism and therapy. The "eight method" of traditional Chinese medicine are designed for the abnormalities of qi. The difference between the unblocking method and the balancing method is only in the focuses, with the former developed for the movement of qi and the latter for the configuration of qi. Both aim to help restore a harmonious state in the human body. This paper presents a detailed analysis of the qi theory, and Prof. Wang’s experience in using Ephedra, Aconite and Asarum Decoction, Minor Bupleurum Decoction, Pinellia Heart-Draining Decoction, Cinnamon Twig Decoction, and Bupleurum and Cinnamon Twig Decoction.

摘要:Professor Wang Qingguo put forward the theory of "achieving harmony by unblocking and balancing" , and applied it to the treatment of endometriosis (EM), an intractable gynecological disease. Traditional Chinese Medicine believes that the pathological nature of EM is blood stasis. The blockage of qi and blood and the imbalance of yin and yang causes abdominal mass, which is one of the critical factors for the disease. In the treatment, Professor Wang advocated that both "removing the blockage to restore the free flow of qi and blood" and "balancing yin and yang" should be emphasized, and pointed out that, for the method of harmonizing yin and yang, "unblocking" and "balancing" are the key, the aim of which is to return to the natural healthy state characterized by the free flows. The "unblocking" method should be used based on the TCM patterns of the patients. The main approaches include warming and dredging and invigorating blood, promoting the movement of qi and removing stagnation, softening and resolving hard masses, and removing blood stasis and lumps. The medicine should be given in appropriate doses, enough to resolve the stasis but not excessive, in order to "achieve harmony by unblocking and balancing" .

摘要:Rheumatoid arthritis (RA) is a systemic autoimmune disease, which is classified as "bi syndrome (arthralgia)" , "lì jié (multiple arthralgia)" and "wāng bì (lame impediment)" in traditional Chinese medicine. Its pathogenesis is mainly wind, cold and dampness arthralgia in the early stage, dampness and heat obstruction or cold and dampness obstruction in the active stage, and binding of phlegm and stasis and deficiency of healthy qi in the remission stage. Prof. Wang Qingguo, following the teaching of Zhang Zhongjing, put forward the academic view of "achieving harmony by unblocking and balancing" , and believed that the disharmony of yin and yang caused by the blocking of evil qi was the common pathogenesis of RA. Given the presence of wind, cold, dampness, heat, phlegm, stasis, and deficiency in all the three stages, the common treatment was established, namely, dispelling wind, dissipating cold, eliminating dampness, clearing heat, removing phlegm, dissolving stasis, and supplementing deficiency. Based on the clinical treatment strategy of dispelling excess evil first and reinforcing healthy qi later, and the relationship between the treatment method, the four-step sequential RA diagnosis and treatment plan of "unblocking, balancing, supplementing, and nourishing" was formulated. Unblocking should be regarded as the basic method and primary treatment principle for RA. In the early stage, the unblocking of wind, cold and dampness is the dominant method, in the middle stage, of dampness, heat, phlegm and stasis, and in the late stage, of phlegm and stasis obstruction. Then, the combination of cold and warm medicinals should be adopted as the main strategy to prevent the recurrence of blocking and obstruction. Since evil qi has been removed, the deficiency is fully revealed, the physician should pay attention to nourishing qi and blood, supplementing the liver and kidney, in order to achieve the purpose of eliminating the disease and consolidate the curative effect. If injuries to the tangible are present, phased conditioning and maintenance are required, in order to return the body to the disease-free balanced state. In this study, the four-step sequential RA diagnosis and treatment plan of "achieving harmony by unblocking and balancing" was explained with clinical records.

摘要:Prof. Wang Qingguo has devoted himself to the research on the clinical differentiation and treatment of spleen and stomach diseases for decades, and developed his ingenious academic insights into the use of classical formulas in treating spleen and stomach diseases. He put forward for the syndrome differentiation and treatment of such diseases that "shaoyang is the pivot of exterior and interior, the spleen and stomach are the pivot of ascending and descending, and clinical diagnosis and treatment should focus on regulating the pivots" . In clinical practice, he focuses on regulating the pivot of shaoyang by invigorating the movement of the pivot, soothing the liver and promoting bile secretion, regulating the pivot of the spleen and stomach by both supplementing and draining, and combination of cold and warm medicinals. It is also needed to differentiate diseases and patterns, and adopt TCM and Western medicine. His most favorite formulas for spleen and stomach diseases are Pinellia Heart-Draining Decoction, Mume Pill, Bupleurum and Cinnamon Twig Decoction, and Bupleurum, Cinnamon Twig and Dried Ginger Decoction.

摘要:Insomnia is a sleep disorder characterized by frequent and persistent difficulty in falling asleep and/or maintaining sleep, resulting in feelings of sleep dissatisfaction. In traditional Chinese medicine, it is known as "bú mèi (sleeplessness)" . Prof. Wang Qingguo put forward the academic idea of "achieving harmony by unblocking and balancing" , and believes that the key to the pathomechanism of the condition is failure to remain in tune with the natural circadian rhythms, which leads to disharmony between yin and yang and disturbance in the functioning of the pivots, thus causing sleep disorders. It is emphasized that shaoyang is the pivot of the exterior and interior, shaoyin is the pivot of fire and water, and the spleen and stomach is the pivot of ascending and descending. In the treatment of insomnia, it is necessary to adjust the pivot of shaoyang, balance the pivot of shaoyin and conciliate the pivot of the middle jiao, and to achieve the state of balance by adjusting the pivots, and harmony by unblocking.

摘要:Perimenopausal insomnia belongs to the categories of "bú mèi (sleeplessness)" and "jué jīng qián hòu zhū zhèng (menopausal symptoms)" in Traditional Chinese Medicine. According to Prof. Wang, the core pathogenesis of perimenopausal insomnia lies in dysfunction of shaoyang acting as a pivot, disharmony of qi and blood, accompanied by disorders of the zangfu organs. It is necessary to comprehensively consider the physiological characteristics of this special stage, grasp the core pathogenesis, and make good use of classical TCM prescriptions. He believes that Bupleurum and Cinnamon Twig Decoction is an important tool of the "harmonizing" methods which regulates qi and blood, ying qi and wei qi, and the three jiao, and restores the entering and exiting at the pivot of shaoyang and the ascending and descending at the pivot of the spleen and stomach. Prof. Wang has developed his unique insights into using Bupleurum and Cinnamon Twig Decoction plus Licorice, Wheat and Jujube Decoction with necessary additions or reductions to adjust the pivot, invigorate the flow of qi and blood, balance the zangfu organs. He may also add single herb or herb pairs to the formula, which creates a synergy to harmonize yin and yang and calm the mind. Furthermore, he attaches great importance to patients’ emotions, and has achieved satisfactory clinical result. His experience of diagnosis and treatment of perimenopausal insomnia was summarized in this study in combination with clinical cases.

摘要:ObjectiveTo observe the effect of Jiedu Xiaoliu Formula (Toxin-Resolving and Tumor-Removing Formula) on progression-free survival (PFS) in patients with B cell non-Hodgkin’s lymphoma (B-NHL).Methods357 patients with B-NHL were divided into the control group (214 cases) and TCM group (143 cases). After propensity score matching (PSM), 118 cases remained in both groups. The PFS, median progression free survival (mPFS), Kaplan Meier cumulative PFS rate and hazard ratio (HR) were observed.ResultsAfter PSM, the mPFS of the TCM group and control group were 76 (45.48, 106.52) months and 50 (28.16, 71.85) months, respectively (χ2=6.82, P=0.009). The Kaplan Meier cumulative PFS rate in the TCM group was higher than that in the control group in two years, with a statistically significant difference (P<0.05). The mPFS of the two groups of low-risk patients with B-NHL in stages I to II was 76 (14.72, 137.28) months in the TCM group and 78 (38.05, 117.95) months in the control group. The difference was statistically significant (P<0.05). In patients with invasive B-NHL, the mPFS in the TCM group was not reached, while that in the control group was 36 (26.73, 45.26) months. The difference between the two groups was statistically significant (P<0.05). Cox regression analysis was used to analyze HR of different subgroups. The result showed that HR of the TCM group was 0.91 (P>0.05) compared with that of the control group. Factors including male, no less than 60 years old, invasive, stage III-IV, B symptoms, elevated LDH, underlying health conditions, ECOGE score of 1-2, radiotherapy and chemotherapy, and DLBCL had statistically significant effects on the risk of progression of B-NHL patients (P < 0.05).ConclusionJiedu Xiaoliu Formula can delay PFS in patients with good physical condition, low-risk stage I-II, and invasive B-NHL.

摘要:Prof. Chai Songyan, laureate of the title of National TCM Master, summarized the physiological characteristics of women at different stages, and proposed treatment strategies for them, in his "Four Most of the Kidney" theory, which can be used as the guiding principles of TCM treatment for female psychiatric patients. For children, because "the kidney is born most early" , before the first menstruation, it is necessary to be cautious in using medicinals that cause frenetic stirring of ministerial fire, and when menstruation is about to come, in using bitter-cold medicinals. For adolescent patients, because "the kidney matures most late" , it is suggested to tonify the kidney and nourish the blood, promote astriction of chong meridian, be cautious in using warm-dryness and bitter-cold herbs. For adult patients, because "the kidney weakens most quickly" , it is needed to supplement the kidney and purge heart fire, but products that break blood should be used cautiously. For the elderly patients, because "the kidney is in most need of protection" , the key is to protect the liver and kidney, adopt both the method of purging and supplementing, and use mild medicinals.

摘要:Minor Center-Fortifying Decoction was first seen in Treatise on Cold Damage and Miscellaneous Diseases. Its basic function is to harmonize ying and wei levels and regulate yin and yang, which makes it suitable for deficiency of qi in the spleen and stomach, excess of wood restricting the soil and disharmony between the liver and stomach. The formula, which takes Yitang (Saccharum Granorum) as the chief medicinal, warms the center and supplements deficiency, reinforces and protects the middle jiao, spleen and stomach. By doing so, it supplements both yin and yang, seeking yang within yin while seeking yin within yang, which is in line with the optimal way of supplementing and nourishing. Professor Zhang Zhiyuan, laureate of the title of National TCM Master, has a unique understanding of clinical idea of Zhang Zhongjing and the use of the decoction. Firstly, He added certain herbal ingredients to the original prescription as needed, which yielded extraordinary results. Secondly, the dosage of individual herbs used in the formula was adjusted according to the pattern of the patient. Thirdly, he also developed unique insights into the precautions of the drug. This study discusses the application of Minor Center-Fortifying Decoction in combination with clinical cases.

摘要:Primary liver cancer refers to malignant tumors produced by hepatocytes or intrahepatic bile duct epithelial cells, which is one of the common malignant tumors in China. With more than 60 years of experience in clinical practice, teaching and research, Prof. Wang Pei has created the ingenious concepts for tumor treatment of "simultaneous internal and external treatment" and "equal emphasis on internal and external treatment" . This combined use of TCM external treatment and internal medicine has been proven effective in the treatment of a variety of difficult cancer diseases. Prof. Wang believes that toxin evil is the key to the pathogenesis of liver cancer, the loss of healthy qi is the internal basis of its incidence, and disease of both qi and blood is the basic pathological change of liver cancer. In clinical practice, the liver and spleen are regarded as the core, with the focus placed on nourishing the yin to emolliate the liver, fortifying the spleen and stomach to reinforce healthy qi, regulating qi and blood to restore the liver functions, and attacking toxin evil to disperse stasis. This study explains Prof. Wang’s academic ideas of emolliating the liver and fortifying the spleen, harmonizing both qi and blood and resolving toxins for the the treatment of liver cancer with clinical records.

摘要:With the advancement of application studies of complex networks approaches, it is found that a single network cannot fully describe and reveal the whole picture and the inherent laws, which gave rise to the theory of hypernetworks. The current hypernetworks are mainly divided into two categories: hypernetwork-based networks and hypernetwork-based hypergraphs. On the basis of defining the hypernetwork and summarizing its modeling approaches and application fields, this study discusses the feasibility of the hypernetwork approach in the integrative diagnosis and treatment for acute cerebral infarction (ACI), and the future research direction of hypernetwork, with the aim to cast some light on the optimization of ACI diagnosis and treatment and related research.

摘要:Coronary microvascular dysfunction (CMD) has become a focus of research in the prevention and treatment of coronary heart disease. Traditional Chinese medicine classifies it as "chest impediment" or "heart pain" , and believes that the pathogenesis of CMD is mostly attributed to heart collateral stasis. The treatment starts from invigorating blood and removing stasis. Qi transformation is the basic form of human metabolism. In broad terms, it refers to all changes and phenomena generated by the interaction of yin and yang in nature. In the narrow sense, it refers to fundamental metabolic processes and biochemical activities and their pathophysiological phenomenon generated during human life activities. Qi transformation is closely related to microvascular endothelial cell function, coronary microcirculatory perfusion, coronary microcirculation hemorheology, and coronary microcirculation inflammatory reaction. Abnormalities in qi transformation can cause changes in hemorheology, inflammatory reaction, endothelial cell dysfunction, etc., and then lead to CMD. The four patterns of heart blood stasis obstruction (excessive transformation of yang), qi stagnation due to congealing cold (insufficient transformation of yang), binding of phlegm and stasis (excessive transformation of yin), and qi deficiency and blood stasis (insufficient transformation of yin) are most closely related to imbalanced qi transformation, which are discussed from the aspects of clinical manifestations, principles of treatment and medication, with the aim to provide new ideas for clinical prevention and treatment of CMD.

摘要:Alzheimer’s disease (AD), a neurodegenerative disease characterized by progressive cognitive impairment, is the most common type of dementia. During the progression of AD, multiple TCM patterns may be present, which follow certain rules. Based on the theory of "the kidney stores the will" proposed in The Yellow Emperor’s Inner Classic, hypothesis of kidney deficiency in AD and its sequential therapy based on kidney-reinforcing was proposed in this study. The therapeutic idea of AD is discussed from the theory of "the kidney stores the will" . If this function is normal, the memory and mental activity remain normal. Abnormalities in the will-storing function of the kidney are not only the initiator of AD, but also the core pathogenesis throughout the whole process of the disease. In light of the close connection between "the kidney stores the will" and the memory and the spiritual activities of human body, the method of supplementing the kidney, producing essence and strengthening the will is proposed as the basic treatment of AD. The focus should be placed on supplementing the kidney in the early stage, invigorating blood, resolving phlegm and draining fire in the middle, and resolving toxins and stopping collapse in the late stage, which provides a theoretical basis for "supplementing the kidney in the early stage and throughout the treatment" in the sequential therapy.

摘要:Nowadays, implantation failure is still a problem to be solved urgently in the application of assisted reproductive technology. Endometrial receptivity represents the role of the endometrium during the implantation window. Endometrium proliferation, secretion transformation is determined by preserving and discharging functions of the uterus. As a subtle substance directly related to reproduction, Tiangui changes with the growth and decline of kidney qi, and is involved in regulating the preserving and discharging functions of the uterus. At the same time, the endometrial receptivity is affected by changes in blood and qi in the uterus. All of these are under the control of the kidney. Kidney deficiency and vascular obstruction of uterine collaterals can affect uterine function, which leads to low endometrial receptivity. Therefore, the method of supplementing the kidney and invigorating blood, removing stasis and clearing collaterals is commonly used for the condition. The types of symptoms should be distinguished in clinical application. The deficiency syndrome is mainly to be treated by tonifying the kidney, while the blood stasis syndrome is mainly to invigorating blood and remove stasis.

摘要:Osteoporotic vertebral fracture (OVF) is the most common type of osteoporotic fracture, for which sarcopenia and osteoporosis are among the high-risk factors. OVF, sarcopenia and osteoporosis fall into the TCM categories of "flaccidity syndrome (wěi zhèng)" , "fleshy flaccidity (ròu wěi)" , and "bone erosion (gǔ shí)" , respectively. TCM believes that the relationship between bones and muscles is in fact the continuation of that between the kidney and the spleen, which further suggests that the normal functioning of the two viscera is the physiological basis of the bone-muscle harmony. Bones and muscles, both stemming from paraxial mesodermal somatic cells, are closely related with each other in structure, function, biomechanics and molecular biology. In the occurrence of OVF, injuries due to falling, tumbling, spraining, or contusion are the inducement, while kidney deficiency is the root, spleen deficiency is the key, and blood stasis is the main pathological product, indicating that spleen-kidney deficiency and blood stasis are the main causes and pathogenesis of OVF. Microscopically speaking, mitochondrial dysfunction leads to insufficient muscle protein synthesis, which manifests in the decline of skeletal muscle mass and contractility, aggravating sarcopenia and osteoporosis. The treatment of OVF by the method of supplementing the kidney, fortifying the spleen and invigorating blood is in line with the principle of prevention before disease onset, controlling development of existing diseases and keeping the disease from recurring after recovery.

摘要:Colorectal Cancer (CRC) is the third most common type of cancer in the world. Traditional Chinese medicine (TCM), based on its theory of reinforcing healthy qi and resolving toxins, has unique advantages in preventing and treating CRC. The key to the prevention and treatment of cancer and its recurrence and metastasis is to strengthen the healthy qi of the patients. This is not only to improve the immune defense, but also to ameliorate the gut microbiota, prevent gene mutation and enhance expression of cancer suppressor genes. It can be even connected with inhibition of tumor angiogenesis, regulation of autophagy of tumor cells and so on. The mechanism of toxins removal of TCM may lie in the improvement of the intestinal environment, reduction of oxidative stress caused by inflammation, prevention of gene mutation induced by inflammation and oxidative stress, inhibition of the proliferation of colorectal cancer cells, and boosting of their apoptosis. Prevention and treatment by TCM based on the theory of reinforcing healthy qi and resolving toxins have been proven to be effective for preventing the canceration of chronic enteritis, reducing the adverse reactions of radiotherapy or chemotherapy, preventing the recurrence and metastasis of CRC and so on.

摘要:Treatise On Blood Syndromes put forward that "to treat all blood syndromes is all about rectifying the liver" , which suggests that differentiation and treatment of blood syndromes should focus on regulating the liver. Myelodysplastic syndrome (MDS) can be classified into the category of "blood syndromes" in traditional Chinese medicine. Based on the idea that the liver is the governor of the free movement of qi, which then guarantees the sound flow of blood, and the liver also stores blood, and well-regulated blood is the prerequisite of health, the treatment method of MDS from the perspective of the liver mainly include: soothing the liver and regulating qi as the core, while invigorating blood and dissolving stasis are the auxiliary; clearing liver heat and draining fire as the core, supplemented with enriching yin and nourishing the blood; clearing the liver and dissolving phlegm as the core, complemented with emolliating the liver and extinguishing wind; supplementing the blood and nourishing the liver as the core, while also warming and moistening to aid qi transformation, so that the liver returns to its healthy state, and the free flow of qi and blood is restored.

摘要:Colorectal cancer is a common type of malignant tumor of the digestive tract that covers the entire section from the rectum to the cecum, and its incidence is increasing year by year. The Case Records as a Guide to Clinical Practice for the first time put forward the concept of "collateral diseases" , to which "abdominal mass" belongs, thus establishing the theory about tumor based on collateral diseases. Colorectal cancer can be classified as "abdominal mass" , "accumulation-gathering" and "intestinal tumor" in traditional Chinese medicine. Based on the theory of collateral diseases, TCM phased treatment and recovery programs are implemented for perioperative management of patients with colorectal cancer: Before the operation, it is key to dispel phlegm and remove blood stasis, resolve toxins and unblock the collaterals, and remove the pathogenic factors in the collaterals. During the operation, the focus should be placed on supplementing qi and nourishing the blood, fortifying the spleen and dredging the bowels, and warming and unblocking the collaterals. After the operation, the priority becomes supplementing the spleen and nourishing the kidney, supplementing weakness and reconciling the collaterals, and removing the residual toxins. Combined with clinical cases, this paper elaborates on the perioperative management of colorectal cancer based on the theory of collateral diseases.

摘要:Lung cancer is one of the major diseases in the world, and intestinal flora is currently a hot topic of research. There is a correlation between intestinal flora dysbiosis and lung cancer, but the underlying mechanism is still unclear. Based on the theory of "the interior-exterior relationship between the lung and large intestine" , this study explores the theoretical foundation, physiological basis, pathological transmission and treatment prescriptions of lung cancer. From the perspective of modern medicine, it is further proposed that intestinal flora can influence the development of lung cancer through inflammation and immunity, and affect the therapeutic effects of chemotherapy and immunotherapy for lung cancer, with the aim to provide new ideas for the treatment of lung cancer.

摘要:Stasis dermatitis is a common peripheral vascular disease, which is characterized by recurrent erythema, papules and itching in the lower extremities, and is easy to diagnose and difficult to treat. The blood and vessel theory is the evidence-based academic viewpoint proposed by Professor Chen Shuchang, laureate of the title of National Prestigious TCM Doctor, in response to the characteristics of clinical diagnosis and treatment of peripheral vascular diseases, emphasizing the holistic nature of the human blood vessel system, with the veins as the core and the wei qi running outside the veins and ying qi inside the veins as the material basis. The etiology and pathogenesis of stasis dermatitis are: deficiency of vessel qi result ing in the loss of propulsion function; stasis in the vessels, blocking the qi flow; dampness in the veins, binding with stasis. The basic treatment method is to boost and reinforce healthy qi to clear the veins, invigorate blood and eliminate stasis to unblock the veins, and clear heat and drain dampness to dredge the veins. The treatment of stasis dermatitis based on the theory of blood-vessel differentiation from the perspective of qi deficiency, damp and stasis is explained in this study with clinical cases.

摘要:ObjectiveTo analyze the herbal medication regularities of prestigious traditional Chinese medicine (TCM) doctors in treating chest impediment and heart pain.MethodsWith the disease "coronary heart disease" in Western medical terms, and the chief complaints including "cardiodynia" and "chest pain" as the keywords, the search was carried out in Prestigious TCM Doctors Inheritance Platform. The time range was from the date of establishment of the database to February 2022. The frequency statistical analysis, association analysis, hierarchical cluster analysis and complex network analysis were conducted with data mining software.ResultsA total of 604 prescriptions meeting the standard were retrieved, involving 6 TCM patterns, with the most common one being heart blood stasis obstruction. 285 herbal ingredients are involved. The first 6 ones with the highest frequency are Danshen (Radix et Rhizoma Salviae Miltiorrhizae), Huangqi (Radix Astragali), Danggui (Radix Angelicae Senensis), Zhiqiao (Fructus Aurantii), Baizhu (Rhizoma Atractylodis Macrocephalae), and Chuanxiong (Rhizoma Chuanxiong). The medicinals are mainly warm and cold in terms of their properties, and the flavors are mainly bitter, acrid and sweet. The most frequently seen meridians entered by the herbs are the liver meridian, the lung meridian and the spleen meridian. As for their functions, the most frequently used are deficiency-supplementing drugs. Association analysis reveals that the drug pairs with the highest support include "Huangqi-Danshen" , "Zhiqiao-Danshen" , and "Baizhu-Danshen" . Cluster analysis was conducted for medicinals with frequency > 30, and three cluster combinations were obtained. The result of the complex network analysis showed that Danshen, Huangqi, Chuanxiong, Danggui, Baizhu, and Zhiqiao were the core nodes.ConclusionThe treatment of chest impediment and heart pain by the prestigious TCM doctors focuses primarily on boosting qi, invigorating blood and dissolving stasis, supplemented by removing phlegm, diffusing impediment and relieving pain.

摘要:Sympathetic chain syndrome is a disease in which various acute and chronic infections, internal and exogenous poisoning, trauma, spinal degenerative diseases, tumors, vascular diseases or other chronic irritating lesions damage different sympathetic ganglia, resulting in symptoms including pain, sensory disturbance, and vascular dysfunction, which may appear in different combinations based on the causes. Clinically, drugs and surgery are often used to relieve pain and block sympathetic nerves. In this study, the method of regulating the heart and calming the mind, clearing heat and dispelling dampness, and unblock the channels and quicken the collaterals was adopted for one case of sympathetic chain syndrome, which was treated with a combined use of heart-regulating and mind-calming acupuncture, conventional acupuncture, and Wonderfully Effective Four Powder. The eventual efficacy achieved was satisfactory, thus providing ideas for the treatment of the condition by traditional Chinese medicine.
