摘要:The TCM master and Professor Xiao Chengzong is the third-generation inheritor of Yanjing Xiao’s gynecology, the fifth batch of representative intangible cultural heritage items in Beijing, which originated from Xiao Longyou, top one of the Four Famous Physicians in the capital city. In the past over 100 years, the academic thoughts, which have been passed down from generation to generation, have been enriched and developed, and have grown up into an academic school specialized in the diagnosis and treatment of gynecological diseases in TCM. Professor Xiao is the direct granddaughter and academic successor of Mr. Xiao Longyou. She has accumulated rich experience in nearly 60 years of clinical, teaching and scientific research work in TCM gynecology, and has exquisite medical skills. She has gradually formed unique academic thoughts. Professor Xiao is a rigorous scholar and insists on tracing the origin of many academic concepts in the field of TCM gynecology. Professor Xiao has inherited and developed the academic thoughts of " Harmony and Flexibility" of Yanjing Xiao’s gynecology, and her prescriptions are based on balance and harmony, and she often selects medicinal herbs with gentle flavors. Prof. Xiao’s approach to gynecology is based on TCM pattern differentiation and treatment, and at the same time, she draws on examination procedures of Western medicine to organically combine pattern-based and disease-based diagnosis and treatment in difficult and complicate gynecological conditions, emphasizing both Chinese and Western approaches. She advocates the inclusion of all, discarding the sectarian views, and actively promotes the exchange and development of different academic schools of TCM gynecology. She emphasizes the importance of teaching and educating people, and is willing to serve as a ladder for others, promoting the construction of a specialized talent team in TCM gynecology. Professor Xiao’s academic thoughts are introduced from various aspects, such as rigorous teaching, tracing the origin, harmony and flexibility, and equal emphasis on TCM and Western medicine, and teaching, etc.

摘要:Professor Xiao Chengzong attaches great importance to the role of the kidney in women’s physiology and pathology. In clinical practice, she differentiates the changes of kidney Qi, kidney Yin and kidney Yang, and based on this, she treats gynecological diseases and creates five methods to treat the kidney. It includes the methods of nourishing the kidney, replenishing the kidney, tonifying the kidney, warming the kidney and consolidating the kidney. The fundamental goal is to achieve a balance between kidney Yin and Yang. The kidney-nourishing method tends to enrich more the kidney, but also takes into consideration the care of the heart and liver; the method of replenishing the kidney tends to tonify both the kidney Yin and Yang; the method of tonifying the kidney tends to supplement kidney Qi to promote the filling of kidney Qi; the method of warming the kidney tends to warm and tonify kidney Yang, but also takes spleen Yang into consideration; the kidney-strengthening method tends to strengthen more the kidney to maintain the stability of the fetus. Professor Xiao’s experience in treating gynecological diseases by using the five methods of treating the kidney is introduced from the content and clinical application.

摘要:Premature ovarian failure refers to amenorrhea, infertility, follicle stimulating hormone>40 IU/L, and decreased estrogen levels in women aged below 40 years, accompanied by varying degrees of perimenopausal symptoms, which is the final stage of ovarian dysfunction. Professor Xiao Chengzong believes that the core pathogenesis of premature ovarian failure lies in kidney deficiency and Thoroughfare Vessel disharmony. Her treatment of premature ovarian failure is based on the principle of tonifying the kidney and regulating the Thoroughfare Vessel. She has prepared Qizi Yishen Lichong Decoction(七子益肾理冲汤) as the basic formula. In clinical practice, she integrates traditional Chinese medicine and Western medicine, and attaches importance to the "real opportunity period" . Once the pregnancy occurs, priority is given to the prevention of miscarriage as early as possible. This paper introduces Professor Xiao’s experience in treating infertility caused by premature ovarian failure with Qizi Yishen Lichong Decoction, Gushen Antai Berverage(固肾安胎饮), etc., in combination with Western medicine. Then Professor Xiao’s diagnosis and treatment and medication ideas are presented through clinical cases.

摘要:Recurrent spontaneous abortion refers to 3 or more consecutive spontaneous miscarriages with the same sexual partner. It belongs to "habitual miscarriages" in traditional Chinese medicine, and is also known as "repeated pregnancies and miscarriages" or "recurrent abortions" . Professor Xiao Chengzong believes that habitual miscarriages are mainly associated with deficiency of the kidney and unconsolidation of the Thoroughfare Vessel and Conception Meridian. Reinforcing the kidney to prevent miscarriages is an important method that Professor Xiao uses to treat habitual miscarriages. In addition, Professor Xiao created an empirical formula, i.e., Gushen Antai Beverage(固肾安胎饮). In clinical practice, she has noticed that the kidney governs reproduction and is the root of the Thoroughfare Vessel and Conception Meridian, and has formed a complete set of sequential therapies, i.e, pre-pregnancy intervention and miscarriage prevention after pregnancy, prevention before disease onset and soothing the liver to relieve Qi stagnation throughout the treatment. Professor Xiao’s diagnosis and treatment ideas for preventing and treating habitual miscarriages and medication experience are introduced in combination with clinical cases.

摘要:Hydrosalpinx refers to fluid-filled distension of the fallopian tubes in the presence of distal tubal occlusion, characterized by female infertility, lesser abdominal pain and abnormal leucorrhea. Professor Xiao Chengzong believes that hydrosalpinx may fall into the category of "Chang Tan" in traditional Chinese medicine. The common causes of this disease are deficiency of Qi and blood, internal injury of seven emotions, and debilitation of Thoroughfare Vessel and the Ren Meridian due to enduring illnesses. Its basic pathogenesis is liver depression and spleen deficiency, interwound phlegm and blood stasis. Professor Xiao believes that simultaneous regulation of the liver and spleen and treatment of both phlegm and blood stasis are the general therapeutic method, and in medication, harmony is the most valued thing. At the same time, both the mind and body should be treated together. New Danggui Shaoyao Powder has achieved good curative efficacy in treating hydrosalpinx. Combined with clinical medical records, this paper introduces Professor Xiao’s diagnosis and treatment ideas and medication experience in treating hydrosalpinx.

摘要:Adenomyosis, a common disease in gynecology which is hard to cure, is caused by the invasion of endometrial glands and stroma into the myometrium, with progressive and aggravated dysmenorrhea as its major clinical manifestation. It falls into the category of "dysmenorrhea" in traditional Chinese medicine according to its clinical manifestations. Master of traditional Chinese medicine Xiao Chengzong believes that the main pathogenesis of dysmenorrhea induced by adenomyosis is kidney Yang deficiency, cold coagulation and blood stasis. Kidney Yang deficiency is considered to be the basis of its pathogenesis, while cold coagulation and blood stasis is the pathological feature. In pattern differentiation and treatment, equal attention should be paid to both warming and unblocking, and to both symptoms and root causes. Professor Xiao’s experience with modified Huluba Pill in treating dysmenorrhea caused by adenomyosis is presented from the following aspects of warming Yang to dissipate cold, moving Qi to relieve pain, transforming blood stasis and unblocking meridians stage-based treatment, strengthening and supporting healthy Qi, and harmonizing Qi and blood. Professor Xiao’s ideas of treatment and medication are presented in combination with clinical medical records.

摘要:ObjectiveTo explore pattern distribution characteristics of chronic cough.MethodsA total of 771 chronic cough patients were included. A clinical questionnaire for TCM syndromes of chronic cough designed by us was used to collect four TCM diagnostic information, including TCM symptoms, signs, tongue and pulse. SPSS 22.0 was used for frequency statistical analysis. Pathological factors, positional factors and syndrome distribution characteristics of chronic cough patients were discussed based on the exploratory factor analysis.ResultsThirty six variables with frequency>8% were selected from 76 information variables of four diagnostic information, then factor analysis was conducted as observation indexes of four diagnostic information. The KMO value was 0.620 (>0.5) and Bartlett ball type test result was F =630, P <0.001, indicating it was suitable for factor analysis. A total of 12 common factors with initial eigenvalue > 1 were extracted. The cumulative contribution rate was 59.368%. The common factor scores of 771 cases were calculated by the regression method. Pathological factors of 771 chronic cough patients were wind (77.43%), dampness-heat (62.13%), phlegm dampness (35.15%), Yang deficiency (29.31%), cold rheum (26.98%) and Yin deficiency (11.41%). Positional factors were the lung (88.98%), spleen (79.77%), stomach (39.43%) and kidney(30.35%). Syndrome distribution characteristics of 771 chronic cough patients were wind hidden in the lung pattern (71.85%), dampness heat retained in the lung pattern (47.86%), phlegm dampness blocking the lung pattern (25.42%), cold fluid hidden in the lung pattern (21.14%), the lung and spleen Yang deficiency pattern (10.89%) and lung Yin deficiency pattern (0.91%).ConclusionThe most common traditional Chinese medicine pattern type of chronic cough is wind hidden in the lung pattern, followed by dampness heat blocking the lung pattern, phlegm dampness blocking the lung pattern, cold rheum hidden in the lung pattern, the lung and spleen Yang deficiency pattern and the lung Yin deficiency pattern. Each pattern can exist alone, but also often coexist with each other. The characteristics of the chronic cough pattern reflect the complicated etiology and pathogenesis of chronic cough. The common pathogenesis of chronic cough is wind hidden in the lung. Pathogenesis characteristics of chronic cough include combination of internal and external pathogenic factors, viscera and bowels interrelated, mixed deficiency and excess. The chronic cough is characterized by a long course of disease, relapsed attacks, protracted and difficult to cure.

      Narrative Medicine

摘要:With the continuous penetration of modern science and technology into medicine, medical staff and patients are calling more and more eagerly for the return of medical humanities, and narrative medicine comes into being. Traditional Chinese medicine contains the germination of narrative medicine, which is rich in medical humanistic thoughts. The connotation of the spirit of benevolence includes four levels: affinity, benevolence, love of things and benevolence. By listening to patients’ narratives, paying attention to neglected emotional factors, and reproducing what patients have said, narrative medicine aims to empathize with patients, establish correlation and belonging relationship, and pay attention to the relationship between people, which is in line with the humane spirit of traditional Chinese medicine. The origin of benevolence comes from the mutual understanding between people of the same kind, which is the idea of empathy emphasized by narrative medicine. On the other hand, the love spirit of self and others has a hierarchy and difference, reflecting the emotional transmission sequence of starting from oneself, and then caring for others from inside to outside, from near to far. The highest level of benevolence is "integrated benevolence" . The essence of the spirit of benevolence and the spirit of narrative medicine share some similarities. Narrative medicine has a rich philosophical theoretical basis, and the spirit of benevolence, based on the traditional wisdom of the Chinese nation, on the basis of the overall natural medical model and thousands of years of practical experience, will play an important guiding role in the localized development of narrative medicine in China.

摘要:Breast cancer is the most common female malignant tumor in China. Invasive breast cancer patients with positive hormone receptors are supposed to receive the adjuvant endocrine therapy after surgery. Tamoxifen and aromatase inhibitors, which are commonly used in the endocrine therapy, both inhibit the growth of cancer cells by suppressing or lowering estrogen levels in the body. A passive decrease in hormone levels can lead to some adverse reactions, such as thickened endometrium, menstrual disorder, hot flashes, vaginal discomfort, fatigue, insomnia, muscle and joint pains, and so on. Such adverse reactions, which are similar to perimenopausal syndrome, are termed perimenopausal-like syndrome, and it belongs to the category of "pre- and past-menopausal syndromes" in traditional Chinese medicine. Professor Lyu Peiwen believes that the manifestations of perimenopausal-like syndrome following the breast cancer surgery are closely related to disharmony between Qi and blood, and the deficiency of the liver and kidney. The main treatment method is to harmonize the Qi and blood and tonify the liver and kidney. The formula is based on "Siteng Er Hong" and Wuzi Yanzong Pill. This paper discusses the theoretical basis of harmonizing Qi and blood, tonifying the liver and kidney from such perspectives as determination of etiologic factors based on differentiation, elucidating etiology based on Qi and blood; multiple symptoms modulated from the liver and kidney; giving consideration to emotion, harmonizing Qi and blood flow. This paper analyzes the prescription of harmonizing Qi and blood, nourishing and modulating the liver and kidney from several aspects: searching for the primary cause of disease in treatment, harmonizing Qi and blood, nourishing and modulating the liver and kidney, modulating essence and blood together, regulating emotions, and soothing the liver and Qi.

摘要:Malignant tumors are common modern malignant diseases which belong to the category of "Jiju" and "Zhengjia" in traditional Chinese medicine. The Yang-warming method is an important method for the treatment of malignant tumors. Yang deficiency leading to cold coagulation is one important disease-causing factor and disease mechanism in the occurrence and development of malignant tumors. From the perspectives of "Yang not being excessive" and "Yang often being excessive" , the lack of true Yang in the human body leads to healthy Qi deficiency, and frenetic stirring of the ministerial fire gives rise to the accumulation of pathological products. When applying the Yang-warming method to treat malignant tumors, on the one hand, its emphasis is laid on seeking Yang within Yin to replenish Yang, on the other hand, the emphasis is on using Yang to warm Yin, so that the deputy fire is filled, and ministerial fire is back to position. Yanghe Decoction is the basic formula used as the Yang-warming method to treat tumors. This formula widely spreads Yang Qi, which has the efficacy of warming Yang to tonify blood, moving Qi and activating blood, dispersing cold to unblock stagnation. It is recorded in Life-saving Manual of Diagnosis and Treatment of External Diseases that Yanghe Decoction treats dorsal furuncle, etc. Dorsal furuncle is closely related to malignant tumors, and dorsal furuncle highly resembles some malignant tumors in etiology, pathogenesis and course of disease. The clinical experience of famous traditional Chinese medicine doctors treating malignant tumors with Yanghe Decoction, and the related clinical studies of Yanghe Decoction treating malignant tumors, all suggest that Yanghe Decoction has good curative effect on malignant tumors. A further understanding of Yanghe Decoction and Yang-warming method is of great significance to the treatment guidance of malignant tumors clinically.

摘要:Fu Qingzhu was a famous doctor in the end of the Ming Dynasty and the beginning of the Qing Dynasty. His masterpiece Fu Qingzhu’s Gynecology has been regarded as the foundation work of gynecology in traditional Chinese medicine. Fu Qingzhu attached great importance to the viscera syndrome differentiation and focused on the liver, spleen and kidney for syndrome diagnosis and treatment. While regulating the liver and kidney, he also focused on the maintainence of the acquired spleen and stomach. The simultaneous treatment of the liver and spleen is based on differentiation of syndromes according to the pathological changes of zang-fu organs and the theory of five elements theory. The simultaneous treatment of the liver and spleen is used throughout the treatment of women’s diseases at various stages of menstruation, leukorrhea, pregnancy and childbirth. Fu Qingzhu’s experience in treating gynecological diseases with the simultaneous treatment of the liver and spleen is analyzed as follows. If spleen disease affects the liver as earth rebels against wood, accordingly, treatment should be invigorating the spleen and soothing the liver. If liver disease affects the spleen as liver deppression affects the spleen, accordingly, treatment should be inhibiting wood and reinforcing earth. If liver disease leads to spleen disease as wood overacts on the earth, the treatment should be restoring Qi to bring adversely risen Qi downward. For simultaneous liver and spleen diseases in which the liver fails to store and the spleen fails to contain, treatment should be tonifying the liver and spleen. Combined with clinical cases, this paper introduces the clinical application of simultaneous treatment of the liver and spleen in gynecology.

摘要:Image thinking is one of the basic thinking modes in traditional Chinese medicine. By combing the concepts of image and image thinking, we aimed to deeply understand the relationship between the thinking mode of traditional Chinese medicine and image thinking. We recognize the ascarid appearance of Jueyin disease from the perspective of image thinking, and the essence of Jueyin disease is the mixture of cold and heat, and the internal movement of wind and Qi. In the process of occurrence and development of bronchial asthma, it is often manifested as the pattern and manifestations of Jueyin disease. Bronchial asthma is clinically characterized by relapsed attacks, diverse triggering factors, time rhythm, seasonality, reversability, etc. From the perspective of image thinking, bronchial asthma resembles Jueyin disease. This paper discusses the treatment of bronchial asthma with Wumei Pill from the aspects of its composition characteristics, clinical application and its mechanism of action. Combined with typical cases, the diagnosis and treatment ideas and medication experience of Wumei Pill in treating bronchial asthma are introduced.

摘要:Sweating is a common physiological and pathological phenomenon in the human body. Pathological sweating is most likely to occur when the balance of Yin and Yang is disturbed and the heart and lungs are out of order. There are many phrases describing sweating in the Yellow Emperor’s Inner Classic, which can be divided into three categories, indicating the state of profuse sweating, the time or cause of sweating, the degree and the prognosis of sweating, all with brief and concise concepts. However, the meaning of "Corporeal Soul (Po) Sweat" has been interpreted differently by medical practitioners throughout the ages, and a unified understanding of its meaning has yet to emerge. The meaning and clinical significance of "Corporeal Soul(Po) Sweat" is discussed in relation to the original text of the Yellow Emperor’s Inner Classic and its understanding and clinical practice by later medical practitioners. "Corporeal Soul Sweat" should be interpreted as a severe degree of sweating, emphasizing the depletion of essence with a poor prognosis. In cases of prolonged sweating or profuse sweating, or even abnormalities of the corporeal soul (Po) such as abnormal limb movements, dulled perception and dysfunction of the organs, the major treatment should be to tonify the essence.

摘要:Gastric cancer is one of the most common malignant tumors in the world, which seriously threatens human health. The treatment of tumors with traditional Chinese medicine has its own unique characteristics. This paper elucidates the explanation of the pathogenesis of gastric cancer in the Principles of Medicine written by Wang Ji, a physician of the Xin’an School in the Ming Dynasty, from its root of "deficiency of middle Qi" and manifestations of "stagnation of dampness-heat" . From the five stages of evolution, the pathogenesis of the gastric cancer is discussed. Deficiency: Deficiency and sinking of Qi and central axis failing to transform are the root of the onset. Cold: Declined spleen deficiency and cold and Yin congestion are the causes of progress. Toxin: Disordered Qi movement and cancerous toxin flow are the core of transition. Closure: loss of control of rising and falling and stagnation of cancerous toxin are the signs of deterioration. Debilitation: Water and fire which fail to support each other and decline of Yin and Yang are responsible for the occurrence of this disease. We explore the syndrome differentiation and treatment of gastric cancer based on the evolution law of the five stages: warming Yang and promoting Qi in the precancerous state and early stage; detoxifying and removing turbidity in the advanced stage and postoperative stage. Combined with clinical cases, the diagnosis and treatment ideas and medication experience of gastric cancer are introduced.

摘要:Childhood dermatosis, one of the common diseases in pediatrics in traditional Chinese medicine, has various etiological factors and multiple patterns. Childhood dermatosis with the pattern of dampness-heat hurting blood vessels has been more common due to improper feeding, inadequate dietary care, etc. The characteristics of childhood dermatosis are analyzed from its diverse etiologies, physiological characteristics, various patterns and dampness-heat hurting blood vessels as its focus. The struggle between dampness and heat, and heat scorching blood vessels are the key pathogenesis of childhood dermatosis with the above-mentioned pattern. In pattern identification, priority is given to the differentiation of skin lesions, etiology, disease staging, viscera, tongue and pulse. In clinical treatment, the physiological and pathological characteristics of children should be taken into consideration while treatment strategies for adult patients are not recommended which are centered on clearing heat and drying dampness, cooling and regulating blood. As for its treatment method, Sanhuang Siwu Decoction is chosen as the main prescription. At the same time, attention is given to skin lesions, integrated internal and external treatment, the disease staging, disease progression, and importance is attached to invigorating the lung Qi, regulating the spleen and stomach, soothing the liver and gallbladder, and clearing and nourishing the heart. Combined with a clinical case, this paper introduces the treatment ideas and medication experience of childhood dermatosis with the pattern of dampness-heat hurting blood vessels.

摘要:Insomnia is characterized by an inability to obtain enough sleep, which is caused by heart spirit with inadequate nourishment and disquieted mind. Yinhuo Tang (YHT) has its origin in Record of Pattern Identification written by Chen Shiduo. It was originally used as a prescription for the treatment of "Yin E" , composed of Shudihuang (Radix Rehmanniae Praeparata), Bajitian (Morindae Officinalis Radix), Fuling (Poria), Maidong (Ophiopogons Radix), Wuweizi (Schisandrae Chinensis Fructus). The pathogenesis of the syndrome treated by YHT is the kidney water deficiency, the frenetic stirring of the ministerial fire, the spleen-stomach disharmony, and the abnormal vaporizing of the triple energizer. After clinical practice and exploration, doctors of later generations expanded the application scope of YHT. It is found that YHT has good effects on insomnia with disharmony of the heart and kidney. The disharmony of the heart and kidney is a common syndrome of insomnia. YHT could nourish Yin for suppressing hyperactive Yang, return fire to the origin, coordinate the heart and the kidney. It is consistent with the pathogenesis of insomnia with the disharmony of the heart and kidney. According to the condition, doctors often modify YHT to strengthen the curative effect, in the treatment of insomnia. Combined with clinical cases, this article introduces thoughts on pattern identification and treatment and medication characteristics of treating insomnia with YHT.

摘要:Cancer-related fatigue (CRF) is one of the most common symptoms among cancer patients. It runs through the entire cancer course and has a serious impact on the life quality of such patients. Traditional Chinese medicine (TCM) has obvious advantages in the treatment of CRF. The literature researches related to CRF of lung cancer were sorted out and analyzed, and the TCM pathogenesis and syndrome manifestations of CRF of lung cancer were discussed. This paper expounds the viewpoint of different researches on the TCM pathogenesis of CRF from two aspects: taking the deficiency as the foundation, taking the excess as the target and mixing the deficiency and the excess. Combined with the literature, the syndrome characteristics of CRF of the lung cancer were summarized in single syndrome and multiple syndromes. Among the single syndromes of CRF of lung cancer, deficiency syndromes are mainly lung-Qi deficiency syndrome and spleen-Qi deficiency syndrome, and excess syndromes are mainly phlegm-dampness syndrome and blood stasis syndrome. The condition of the patients is usually complicated, and therefore only a few patients with mild fatigue or early-stage lung cancer show a single syndrome. Most of them show multiple syndromes.
