      摘要:Narrative medicine, which emerged in the United States in 2001, is a new branch of medical humanities. It expresses empathy and reflection of medical humanities in language through reading and writing. Traditional Chinese medicine contains rich humanistic elements. As important chapters in Huangdi Neijing, Shu Wu Guo Lun and Zheng Si Shi Lun pointed out the five failings and four lapses that healers may make. In addition to profound medical theories, there is also rich humanistic reflection to be found in the two chapters. From the perspective of etiology, the researchers analyzed the reflections on the changes of social life and major negative life events, on the emotions, on lifestyle of diet and residence, and on personalities and constitutions. From the point of reflection on the doctors themselves, this paper analyzes the reflection on the poor mastery of medical science, on the lack of concentration on the medical profession, and on the psychology of the thoughtless, arrogant and greedy doctors. The two pieces are full of reflections on medicine and have the characteristics of intersubjectivity, empathy and reflection of narrative medicine, which can enlighten the local development of narrative medicine.

      摘要:ObjectiveTo explore the perceptions and attitudes of 50 acupuncturists in China on the factors influencing cupping colors and the relationship between the colors and efficacy.Methods60 acupuncturists who participated in a training program were invited to join in this survey. A questionnaire containing twelve questions covering 6 domains was used to understand the participants’ attitude and understanding of the influencing factors and significance of cupping colors.Results53 acupuncturists signed the informed consent and filled the questionnaire, 3 of them were excluded due to the missing of key information. A total of 50 valid questionnaires were included in the analysis. Half of the respondents have senior professional titles, of which 36% were deputy chief physicians. According to the 95% confidence interval of the population proportion, more than 50% of the acupuncturists believe that the negative pressure, retention time, the health status of the recipient, traditional Chinese medicine (TCM) body constitutions, and TCM pattern type are possible factors affecting the color change of cupping marks. In addition, the color may be related to the severity of diseases, especially locomotor and respiratory diseases. The color may also reflect the therapeutic effect of cupping. Five respondents mentioned that the color tends to become lighter with the continuous progress of treatment and the relief of symptoms. There was no significant difference in all these views among subgroups with different professional titles and ages.ConclusionFive factors may affect the color of cupping marks. The color may not only reflect the severity of the disease but also have an important connection with the clinical curative effect. However, due to the small sample size of the survey, a larger clinical trial is needed to further explore the relationship between cupping colors and the treatment efficacy.

      摘要:ObjectiveTo use bioinformatics method to identify potential therapeutic targets, biological processes and related pathways of repeated implantation failure (RIF), and to predict Chinese herbal remedies for the genes.MethodsThe RIF endometrial tissue microarray dataset was downloaded from the GEO database, and R software was used to identify differentially expressed genes (DEGs). Enrichment analysis was performed to construct protein-protein interaction (PPI) networks to understand the functions and enrichment pathways of DEGs and to identify key genes. Finally, Coremine Medical analysis was used to identify herbal medicines acting on RIF key genes.ResultsA total of 250 differential genes were screened out. These DEGs were involved in the regulation of biosynthesis of organic cyclic compounds, positive regulation of nitrogen compound metabolism, positive regulation of cell metabolism, cell activation, bioadhesion, leukocyte activation and other biological processes. It was associated with chemokine signaling pathway, apelin signaling pathway, neuroactive ligand-receptor interaction, etc. The key genes identified include PTGS2, AGT, VCAM1, C3, FGB, RBP4, IGFBP1, PLA2G10, MUC1, GJA1, ACTA2, LPAR3, ABCG1, HABP2, CYP26A1, FETUB, GPX3, PTGS1, PLA2G2A, and NR4A2. TCM herbs found to act on the genes were Renshen (Radix et Rhizoma Ginseng), Roucongrong (Herba Cistanches), Huangqi (Radix Astragali), Shanyao (Rhizoma Dioscoreae), Shayuanzi (Semen Astragali Complanati), Tusizi (Semen Cuscutae), Danshen (Radix et Rhizoma Salviae Miltiorrhizae), and Sanqi (Radix et Rhizoma Notoginseng).ConclusionsBy screening the key genes of RIF and predicting potential therapeutic Chinese herbal medicines, this study provides important strategies and directions for future treatment.

      摘要:ObjectiveTo analyze the efficacy of acupuncture on malnutrition in patients with peritoneal carcinomatosis from colorectal cancer.MethodsEighty-four patients were randomly divided into a treatment group and a control group, with 42 in each. The treatment group was intervened by acupuncture plus standardized nutritional treatment, while the control group was given standardized nutritional treatment. Patient-generated subjective globe assessment, the ingestion improvement rate, body mass index, prealbumin andalbumin were evaluated to measure the therapeutic efficacies after 6 weeks.ResultsThe score of the patient-generated subjective globe assessment in the treatment group was significantly lower than that before treatment, while ingestion improvement rate, body mass index, prealbumin and albumin increased compared with those before treatment after 6 weeks(P<0.05). The treatment group was superior to the control group (P<0.05).ConclusionAcupuncture treatment could improve malnutrition in patients with peritoneal carcinomatosis from colorectal cancer.

      摘要:The theory of traditional Chinese Medicine (TCM) Constitutions has made considerable progress in the past decades. Belonging to one of its three key scientific issues is "Adjustability of TCM Constitutions" . Using evidence-based medicine approaches, this article explored the keypoints that should be paid attention to in the clinical research for evaluating the effectiveness of TCM constitutional adjustment regimens from the following aspects: participants selection, design and implementation of the regimens, selection of indicators, and selection of research types. The aim is to provide a basis for the objective, scientific and comprehensive evaluation of the intervention efficacy and safety of the TCM constitution regimens, and to promote the international development of the theory of TCM constitutions.

      摘要:In recent years, there has been growing interest in research on "gut microbiota-intestine-brain" axis in modern medicine. Many studies found that gastrointestinal microbiota regulation is beneficial for the treatment of brain-related diseases. In traditional Chinese medicine (TCM), the brain and gastrointestinal system are connected through meridians, and they are closely related both physiologically and pathologically. In the current society, the liver constraint and spleen deficiency pattern has become one of the dominant types of insomnia. Research has found that the liver and spleen in TCM terms are closely related to the nervous, endocrine and immune systems in modern biology, which correspond to the "gut microbiota-intestine-brain" axis and its three pathways. Based on TCM theory and the theory of "gut microbiota-intestine-brain" axis, we proposed that insomnia caused by liver constraint and spleen deficiency is closely related to the "gut microbiota-intestine-brain" axis, the dysfunction of which might be its potential biological mechanism.

      摘要:Based on Professor Wang Qi’s "three differentiation" model, the diagnosis and treatment of varicocele (VC)-induced infertility is a three-step process: disease differentiation as the first, followed by constitution differentiation in combination with pattern differentiation, thus concluding that the fundamental etiology of VC is "internal obstruction of static blood" and the main pathogenesis of VC-induced infertility is "static heat in blood aspect damaging yin and kidney essence depletion" . The differences in pathomechanism and pattern of VC-induced infertility with different constitutions were also revealed. The primary constitution of VC patients is blood stasis, while others include yin deficiency, yang deficiency, dampness-heat, qi deficiency, and qi constraint. For VC-induced infertility, internal obstruction of static blood is the main pattern, most commonly complicated by patterns of yin deficiency of of both liver and kidney, kidney-yang deficiency, dampness-heat in semen chamber, both qi and blood deficiency, liver constraint and qi stagnation. The treatment plan for VC-induced infertility is formulated as guided by Professor Wang’s theory of "one major formula for one disease" . The therapeutic principles and method are activating blood and discharging heat, removing blood stasis and dredging collaterals, nourishing yin and promoting essence. Danhong Tongjing Prescription (DTP), an original formula, is created as the main remedy. According to the "four-factor" model, the treatment plan is flexibly applied in the clinical diagnosis and treatment of VC-induced infertility, and corresponding treatment methods and prescriptions are applied to various complicating patterns. Comprehensive understandings of the nature, onset regularity, and pathological characteristics of the disease from different levels of constitution, disease, and pattern, as well as flexible design of clinical prescriptions can provide new ideas and more suitable treatment plans for the diagnosis and treatment of the disease.

      摘要:Acute gout is a kind of metabolic rheumatism that induces local joint inflammation and tissue destruction by factors such as drinking, high-purine diet, sudden cold and strenuous exercise. Its mechanism can be described with the theory of "spleen diseases leading to limb disorders" . Acute gout belongs to the category of Tongfeng Bing in traditional Chinese medicine. It is caused by the overconsumption of fatty, sweet, heavy, and alcoholic foods and drinks, irregular lifestyle, excessive work or sports, which lead to dysfunction of spleen and stomach, and damp, heat, phlegm, fire, and stasis blocking meridians. In addition, the joint pain is also caused by wind, cold and dampness invading the meridians and vessels, resulting in the blockages. Therefore, it is suggested to apply acupuncture at the five transport acupoints and traditional herbal medicine to dredge the meridians based on reconciling the spleen and stomach, so as to relieve the pain in the limbs.

      摘要:Drug pairs are the minimum fixed combination unit of the composition of traditional Chinese medicine(TCM) formulas, which can play a synergistic effect, reduce toxicity and side effects.Huangqin (Radix Scutellariae) and Zhizi (Fructus Gardeniae) are one of the representative drug pairs for clearing heat, purging fire and removing toxin, which are widely used in clinical scenarios.Each of the herbal ingredients used alone has the effect of clearing heat and draining fire, and combination of the two can drain fire and calm the heart, clear heat and purge the lung, remove toxin and protect the brain. In these ways, fire and heat in the upper jiao can be completely relmoved. In this paper, we systematically reviewed the cases of the combination of Huangqi and Zhizi in the treatment of upper jiao disorders, comprehensively analyzed the theoretical basis of the herbal pair, in order to expand the scope of its clinical application, and provide ideas for the study of drug pairs of traditional Chinese medicine.

      摘要:Chronic prostatitis (CP) is a common disease in urology and andrology, which is a tricky disease due to its unknown etiology, complicated symptoms and prolonged course. At present, it has become a new direction of clinical research to explain the etiology, pathogenesis and treatment of this disease from the perspective of deficiency of the spleen and stomach based on the physiological characteristics of the two as the foundation of acquired constitution to nourish all viscera. With the research advancing, the relationship between gut microbiota and the spleen and stomach and CP has been gradually uncovered. For example, gut microbiota is similar to the spleen and stomach in physiological function, pathogenic factors, symptom manifestations, pathological state and medication regulation. CP is closely related to spleen deficiency in terms of disease name, symptoms, pathogenesis, treatment and prescriptions. Moreover, gut microbiota can induce CP through nerve, endocrine and immune regulation. Based on the relationship between the three, we believe that spleen deficiency leads to gut microbiota imbalance and thus induces CP, which can not help explain its pathogenesis, but also be used as scientific support for treating the disease from the perspective of the spleen and stomach.

      摘要:This study collected literature relevant to "mù dé guì zé kū (wood withers with the presence of cassia)" in Chinese Medical Code and presented the most representative contents in combination with clinical practice and discussion of modern physicians.It is concluded that Rougui (Cortex Cinnamomi) and Guizhi (RamulusCinnamomi)are sweet and acrid in flavor and tends to wander. Acrid metal can restrict redundant wood, and the wandering actions of the herbs can relieve wood constraint. In addition, the effect of calming surging and directing counterflow downward of both herbs indicate that they can bring the chaotic and reverse qi movement back to order. Guizhi and Rougui can both adjust the liver in a two-way mode by soothing and calming it. While calming and inhibiting the liver, they can also restore the free flow of qi, acting in a boosting and suppressive manner, which is preferred by the liver. Furthermore, the sweet-warm medicinals can supplement earth, enabling the use of one herb to soothe the liver and fortify the spleen.The analysis and summary of ancient and modern literature is of great significance to expand the application range, explore the combination principles of classic formulas, simplify clinical prescriptions and enrich the use of drugs for liverdiseases.

      摘要:The palmprint diagnosis is a method of diagnosing diseases by observing the lines and wrinkles of the palm of a hand, which is an important part of TCM hand diagnosis. Important auxiliary method as it is in clinical practice, palmprint diagnosis has not been systematically and adequately studied in academic circles. This paper discusses the contents and diagnostic characteristics of representative schools of palm print diagnosis, such as the infantile fingerprint diagnosis, nine palaces and eight trigrams hand diagnosis, palmistry mount diagnosis, dermatoglyphics, etc, and expounds on the theoretical influence of these schools on hand diagnosis, in order to promote the inheritance and development of hand diagnosis and provide assistance for researchers.

      摘要:This study explains the pathogenesis, syndrome differentiation and treatment of cheilitis from different kinds of pathogenic fire. The basic pathogenesis process of cheilitis in Traditional Chinese medicine (TCM) is heat brimming in the heart, spleen and stomach followed by fire-pathogen invading upwards. Based on the clinical manifestations and pathological features of cheilitis, this paper puts forward that the source of pathogenic fire can be divided into sthenic fire, ministerial fire and yin fire. The TCM pathology of cheilitis is explained from the perspective of pathogenic fire, including brimming heat in the heart, spleen and uprush of sthenic fire; yin deficiency and blood dryness and frenetic stirring of ministerial fire; spleen and stomach deficiency and upward flaming of yin fire. Some treatment method are also proposed here, which include the use of Xiehuang Powder (Yellow-Draining Powder) or Shuangjie Tongsheng Powder (Double-Resolving Sage-Inspired Powder) to clear the heart and spleen and drain the sthenic fire; Qingxin Lianzi Beverage (Heart-Clearing Lotus Seed Beverage) or Sancai Fengsui Pellet (Heaven, Human, and Earth Marrow-Retaining Elixir) to nourish yin and benefit qi and clear ministerial fire; Bupiwei Xieyinhuo Shengyang Decoction (Stomach-Spleen-Supplementing, Yin-Fire-Draining, and Yang-Raising Decoction) or Buzhong Yiqi Decoction (Center-Supplementing and Qi-Boosting Decoction) to supplement the spleen and stomach and disperse yin fire. These are designed to achieve its desired effect of eliminating pathogenic fire.

      摘要:ObjectiveTo explore the characteristics of Ye Tianshi’s differentiation and treatment of external pathogen constraint.MethodsBy using the method of literature research and data analysis, we screened the medical records related to the constraint of external pathogensin the Case Records as a Guide to Clinical Practice (Lín Zhèng Zhǐ Nán Yī Àn), analyzed the principles, methods prescriptions and medicinals involved, and summarized the treatment characteristics of Ye Tianshi.ResultsThe total number of cases of constraint caused by external pathogenic factors in the book was 458, with the top three most frequently seen types of pathogens being heat constraint (27.5%), damp constraint (24.5%), and fire constraint (22.9%).Wind constraint is usually treated with acrid medicinals with a dispersing effect, cold constraint with acrid medicinals with a dispersing effect and warming and dispersing. For heat constraint, the heat clearing method is used if heat pathogen is dominant, and the dispersing method is used for the type characterized by severe heat constraint. For damp constraint, the dispersion method is often used. Dryness constraint is often treated with acrid-cool and sweet-moistening herbs. For fire constraint, it is needed to relieve heat and nourish yin method.ConclusionFor the treatment of the constraint of external pathogens, Ye Tianshi’s main idea of syndrome differentiation and treatment was to useacrid medicinals with a dispersing effect, to diffuse and unblock, and to dispel external pathogens, with special attention given to the constitutions.

      摘要:As the representative figure of the Yi Shui school, Li Dongyuan is famous for his theory of fortifying spleen and stomach. Li repeatedly mentioned the rule of "presenting the scenes of spring and summer (xíng chūn xià zhī lìng)" in his works. In order to explore the meaning, medication characteristics and clinical significance of this theory, this paper collects and analyzes the relevant sentences and finally comes to the conclusion that Li’s theory responds to the rise of grain qi and gallbladder qi and the growth of yang qi in the human body, which is the basis of human physiological activities. Therefore, he attached importance to supplementing qi and ascending yang in the treatment. In terms of medication, he focused on the use of acrid and sweet herbs with a dispersing effect. Li applied the method of supplementing qi and ascending yang to treat sinking diseases such as metrorrhagia and diarrhea, as well as disorders related to upper part of human body such as headache and eye diseases, which has an important impact on physicians of later generations.

      摘要:Alzheimer’s disease (AD) is a neurodegenerative disease characterized by cognitive impairment. Although the term cannot be found in traditional Chinese medicine (TCM) classics, the disease has long been recorded, providing a solid theoretical foundation and rich clinical experience. In the modern era, various TCM treatment methods have also been put forward for it, including supplementing the kidney, dissolving phlegm, and invigorating blood.However, for AD, a dynamically progressing disease, the principle of "combining diseases in modern medicine and TCM patterns" should be adopted, and stage-based syndrome differentiation and treatment in a targeted manner is key. In terms of disease characteristics, the core and throughout pathogenesis is kidney deficiency, especially in the early stage. With the disease progression, excess pathogens like phlegm, blood stasis, and fire stand out in the middle stage. In the late stage, the exuberance of toxin with decline of healthy qi accelerates the deterioration of the patient’s conditions. As for individualized syndrome differentiation, the theory of "five viscera storing spirit" can be applied for in-depth identification of cognitive, psychological and behavioral symptoms in addition to finding the overall pattern based on zang-fu pattern differentiation. This study puts forward a four-level TCM therapy of AD from the perspective of "combining diseases in modern medicine and TCM patterns" : 1) supplementing the kidney as early as possible and throughout the stages; 2) adopting the method of resolving phlegm, dispelling stasis, purging fire, removing toxin and preventing collapse at different stages; 3) taking a global approach based on syndrome differentiation of zang-fu viscera; 4) adopting the theory of "five viscera storing spirit" for cognitive, psychological and behavioral symptoms.

      摘要:ObjectiveWe searched for Crohn’s disease(CD) related genes through bioinformatics data mining, and used network pharmacology methods and molecular docking techniques to investigate the targets and potential mechanisms of action of Tuoli Xiaodu Powder (Vital Qi Strengthening and Toxin Removing Powder) for the treatment of CD.MethodsThe genes related to CD were obtained from the GEO database, Online Mendelian Inheritance in Man (OMIM) and GeneCards, and the active compounds and corresponding targets of the components of the Tuoli Xiaodu Powder were identified in the TCMSP database and the UniProtdatabase, and the disease-formula regulatory network was created with Cytoscapes.The PPI network was constructed for the crossover genes between the disease and the prescription and the core gene targets were identified.ResultsA total of 922 CD related genes were obtained after deduplicating, 241 active compounds related to Chinese herbal compound, 123 compound-related genes were obtained by Durgbank proofreading of the corresponding targets, and a total of 23 disease-formula crossover genes were selected by network pharmacology, and the core compounds selected by network pharmacology were quercetin, luteolin, fisetin, kaempferol, and licochalconeA.The core gene targets identified by PPI network map were ALB, IL6, ESR1, PPARG, VEGFA, and EGFR.The GO analysis of the crossover genes suggested that the genes were enriched in the regulation of DNA-binding transcription factor activity, regulation of lipid storage, etc., and the KEGG pathway was enriched in multiple cancer, TNF, and NF-κB pathways; molecular docking validation showed good binding ability of compound ligands to protein receptors.ConclusionThe ingredients of Tuoli Xiaodu Powder may act on a variety of cancer, TNF, and NF-κB pathways through related receptor proteins to regulate inflammatory, immune mechanisms to control the development of CD.

      摘要:ObjectiveTo explore the distribution of main treatment syndromes, law of addition of herbal ingredients and dose-effect relationship of Gegen Qinlian Decoction (Pueraria Root, Scutellaria Root, and Coptis Decoction), and to provide basis for modern application of the formula.MethodsThe main symptoms, prescription composition and dosage were collected from CNKI, Wanfang database and VIP database and statistically analyzed, and EXCEL was used to establish a database, and medication rule and dose-effect relationship were explored.ResultsGegen Qinlian Decoction can be administered to both adults and children, the syndromes it can be used to treat were diarrhea (also in children) and diabetes, and the administration method mainly included oral administration and enema. When the medicine is ingested orally by adults, the herbal ingredients added to the original formula are, in descending order of frequency, Fuling (Poria), Baizhu (Rhizoma Atractylodis Macrocephalae), Muxiang (Radix Aucklandiae), Baishao (Radix Paeoniae Alba), and Chenpi (Pericarpium Citri Reticulatae), etc.; for adult enema, Diyu (Radix Sanguisorbae), Huaihua (Flos Sophorae), Baishao (Radix Paeoniae Alba), Baizhu (Rhizoma Atractylodis Macrocephalae), and Baijiangcao (Herba Patriniae); for oral administration to children, Fuling (Poria), Baizhu (Rhizoma Atractylodis Macrocephalae), Cangzhu (Rhizoma Atractylodis), Cheqianzi (Semen Plantaginis), and Liushenqu (Massa Medicata Fermentata). Addition or reduction of these herbs are generally made as necessary. The most common version of the original formula consists of Gegen (Radix Puerariae Lobatae) 10-15 g (5-10 g in children), Huangqin (Radix Scutellaria) 6-10 g (3-6 g in children), Huanglian (Rhizoma Coptidis) 6-10 g (3-6 g in children), Gancao (Radix et Rhizoma Glycyrrhizae) 6-10 g (2-6 g in children). There is a dose-effect relationship between the syndromes and dosage. In the treatment of type-2 diabetes, ulcerative colitis, acute gastroenteritis/acute enteritis, the usual dosage of Gegen was 30 g, 10-15 g, 10-15 g, respectively, Huangqin 12-20 g, 6-10 g, 6-10 g, Huanglian 6-10 g, 6-10 g, 6-10 g, and Gancao 5-15 g, 6-10 g and 6 g. The most widely used ratio of the basic ingredients is currently 5∶3∶3∶2. The syndromes that it is effective for may vary according to different ratios between two herbs.ConclusionAnalysis of medication rules and dose-effect relationship of Gegen Qinlian Decoction can provide basis for clinical practice and further pharmacological research.

      摘要:This study is a comprehensive explanation of Prof. Tao Guangzheng’s experience in syndrome differentiation and treatment variation of tumors. Prof. Tao devised the five principles of TCM treatment of tumors based on his clinical experience. First, reinforcing healthy qi and dispeling pathogen are the primary rule, and the ratio of the two methods should be adjusted in a dynamic balance according to the patient’s physical conditions. Second, harmonizing yin and yang is the next most important task, the aim of which is to make sure yin is calm and yang is sound, and that yang can better play the leading role in normal life activities, so that yin and yang are interdependent and mutually beneficial. Third, it is necessary to restore the free flow of qi and blood, regulate the spleen and stomach, mediate the middle jiao, and make the ascending and descending of qi smooth and well-regulated. Fourth, the attacks on the pathogen should not be excessive. Desirable results can only be achived with accurate syndrome differentiation and treatment variation. Fifth, emotional and dietary maintenance are key, both of which are also important factors affecting the progress of the disease. The combination of these five method and the combination of syndrome differentiation and disease differentiation has important value for the prevention and treatment of tumors and the recovery of the patients. It is also of great significance for the improvement of the clinical efficacy of traditional Chinese medicine in the treatment of tumors.

      摘要:Gastric cancer is one of the common malignant tumors of the digestive tract, with a high incidence and fatality rate in China. Since it is difficult to be detected at early stages, most of the cases are already in the advanced stage when the diagnosis is made. However, the therapeutic effect for advanced gastric cancer is usually poor, and it is easy to progress and relapse, and the survival time is short. Prof. Liu Shenlin, laureate of the title of National Prestigious TCM doctor, is good at the treatment of spleen and stomach diseases, and has unique views on the treatment of advanced gastric cancer.He believes that advanced gastric cancer is mostly caused by lack of healthy qi enabling invasion of cancerous toxin, and weakness of the spleen and stomach leading to the loss of their function of controlling transportation and transformation, so that qi deficiency causes blood stasis. With time going by, the blood stasis and cancerous toxin bind with each other and give rise to the cancer.Hence, Prof. Liu proposed that, for advanced gastric cancer, the pathomechanism of which is spleen deficiency and stasis toxin, it is necessary to adopt the method of boosting qi and fortifying the spleen, dissolving stasis and removing toxin as the basic treatment principle.In addition, after the fundamental idea of treatment is decided based on pathogenesis, clinical differentiation and treatment should then start from the syndrome differentiation of zang-fu organs, combined with the syndrome differentiation of eight principles, emphasizing the holistic treatment, paying attention to the properties, tastes, and channel tropism of herbal ingredients, selecting drugs appropriately, using drugs carefully, so as to guarantee significant clinical efficacy.In this paper, we introduce Prof. Liu’s experience in the treatment of advanced gastric cancer in combination with a clinical case record.
