摘要

目的

观察乌梅丸加减方外用治疗维持性血液透析皮肤瘙痒患者的临床疗效。

方法

将70例维持性血液透析伴皮肤瘙痒的患者随机分为治疗组35例、对照组35例，剔除脱落病例后治疗组32例、对照组34例。治疗组、对照组均予规律血液透析和常规基础治疗，对照组给予炉甘石洗剂外用，治疗组予乌梅丸加减方外用。观察2组瘙痒视觉模拟评分、改良Duo’s瘙痒评分、匹兹堡睡眠质量指数(PSQI)总分、临床疗效。

结果

2组瘙痒视觉模拟评分治疗后较本组治疗前均降低(

0.05)，治疗组较对照组降低明显(

0.05)，治疗组优于对照组。2组改良 Duo’s 瘙痒评分治疗后较本组治疗前均降低(

0.05)，治疗组优于对照组。2组临床疗效比较，治疗组32例中显效8例，有效22例，无效2例，总有效率为93.75%；对照组34例中显效4例，有效21例，无效9例，总有效率为73.53%。经秩和检验，

0.05，说明治疗组临床疗效优于对照组。

结论

乌梅丸加减方外用可有效减轻维持性血透患者皮肤瘙痒症状，改善睡眠质量，有较好的临床疗效。

Abstract

Objective

To observe the clinical efficacy of externally-applied and modified Wumeiwan Formula for pruritus in patients with the maintenance therapy of hemodialysis.

Methods

Seventy maintenance hemodialysis patients with pruritus were randomly and equally assigned to the control (

=35) and treatment groups(

=35). There were 32 left in the treatment group and 34 left in the control group

with the withdrawal or drop-out excluded. On the basis of hemodialysis and conventional treatment

calamine lotion was applied to the control group while the modified Wumeiwan Formula was given to the treatment group as the topical treatment. The visual analogue scale

Duo’s pruritus score

Pittsburgh sleep quality index and total effective rate of treatment were evaluated before and after the treatment.

Results

The score of the visual analogue scale statistically decreased after the treatment in both groups(

0.05)

with a significantly greater difference in the treatment group than in the control group(

0.05). Duo’s pruritus score significantly decreased after the treatment in both groups(

0.05)

with a significantly greater difference in the treatment group than in the control group(

0.05). The total score of the Pittsburgh sleep quality index was statistically decreased after the treatment in both groups(

0.05)

with a significantly greater difference in the treatment group than in the control group(

0.05). As for the clinical efficacy

8 cases showed remarkable effects

22 cases were effective and 2 cases were ineffective

with a total effective rate of 93.75% in the treatment group. In the control group

4 cases had remarkable effects

21 cases were effective

9 cases were ineffective

with a total effective rate of 73.53%. Nonparametric test showed that as compared with the control group

the modified Wumeiwan Formula has advantages in clinical effects (

Conclusions

The externally applied modified Wumeiwan Formula demonstrates remarkable therapeutic efficacy in relieving pruritus and improving their sleep quality in patients with maintenance hemodialysis.
