摘要

特发性耳鸣成人患病率较高，根据病程可分为急性耳鸣和慢性耳鸣，急性耳鸣可伴有听力下降、耳闷堵感、听觉过敏等，慢性耳鸣可伴有失眠、不良心理状态等。肝胆火旺是急性耳鸣的主要证候，治以清泻肝胆、通利耳窍，应用“通耳方”联合耳尖或耳后放血疗法，其中耳周腧穴应用火针。肾精不足是慢性耳鸣的主要证候，治以补益肝肾、镇静安神，应用“聪耳方”为主，其中耳周腧穴亦应用火针。特发性耳鸣病因、机制不清，应详细探查可能诱发或加重耳鸣的相关因素，进行针对性针灸治疗以改善耳鸣。

Abstract

Idiopathic tinnitus is more prevalent among adults

which can be divided into acute and chronic tinnitus according to the course of disease. Acute tinnitus may be accompanied by hearing loss

ear congestion

auditory hypersensitivity while chronic tinnitus may be accompanied by insomnia

adverse psychological state and so on. Flaming fire of the liver and gallbladder characterizes acute tinnitus. Treatment for this pattern is centered on clearing away the liver and gallbladder and unblocking the auricular orifices

and "Tong’er Formula" in combination with bloodletting therapy at the ear tip or behind the ears should be applied to treat the acute tinnitus. Acupoints around the ear should be treated with fire needles. Insufficiency of kidney essence characterizes chronic tinnitus. The treatment is mainly to tonify the liver and kidney

calm the nerves and to use "Cong’er Formula"

among which acupoints around the ear are also used with fire needles. The etiology and mechanism of idiopathic tinnitus are not known. The relevant factors that may induce or aggravate tinnitus should be explored in detail

and targeted acupuncture treatment should be carried out to improve tinnitus.
