摘要

IgA肾病是临床上最常见的原发性肾小球疾病，反复发作的肉眼血尿或镜下血尿、蛋白尿是IgA肾病主要的临床表现。中医将其归属为“肾风”“尿血”“水肿”“尿浊”等范畴，并认为该病以脾肾亏虚为本，风湿毒为标，瘀血贯穿疾病始终。对IgA肾病从瘀论治的病理机制进行探讨，从益气活血、标本兼治，清热凉血、热去络宁，活血利水、斡旋三焦，祛瘀解毒、推陈致新等方面论述用活血化瘀法治疗IgA肾病，并结合临床病例对具体用药进行介绍。

Abstract

IgA nephropathy is the most common primary glomerular disease in clinical practice

and recurrence of gross hematuria or microscopic hematuria and proteinuria are the main clinical manifestations of IgA nephropathy.Traditional Chinese medicine classifies it into "kidney wind"

"bloody urine"

"edema"

"urine turbidity" and other categories

and it holds that this disease has the spleen and kidney deficiency as its root cause

and wind

dampness and toxicity as its symptoms and blood stasis runs throughout the whole disease course.This paper explores the pathological mechanism of IgA nephropathy from the perspective of blood stasis and investigates its treatment from invigorating qi and promoting blood circulation

including replenishing qi to activate blood

treating symptoms as well as root causes

clearing heat and cooling blood

removing heat and calming collaterals

promoting blood circulation and disinhibiting water

regulating Sanjiao

removing blood stasis and detoxification

pushing the old and bringing the new. Besides

this paper also introduces the use of medicinals in combination with clinical cases.
