复发性尿路结石是一种慢性代谢性疾病,以脏腑紊乱为核心,以湿热留滞为主要表现形式,病理特征为气血凝滞,主要病机为相火下陷膀胱、三焦气化失常和血气凝滞。临床可分期论治,急性期属湿热下注、气血逆乱,治以利湿理气祛瘀;术后期或清石后期水湿留滞兼夹瘀浊,治以轻用通利、兼理气血。排石后期病机特点为湿浊有余化热,而兼脾肾不足,治法以调脏腑、和阴阳、化湿浊为主。此外,饮食和生活习惯调摄是防治尿石症的重要方面。
Recurrent urinary calculi are a chronic metabolic disease with disturbance of the
zangfu
organs as the core
dampness-heat retention as the main manifestation
and the pathological feature is stagnation of qi and blood. Its main pathogenesis is ministerial fire sunken into the bladder
disorder of qi transformation at the three
jiao
and qi stagnation and blood stasis. Clinical treatment should follow a staged approach: In the acute phase
the focus is to address damp-heat pouring downward and counterflow of qi and blood. The treatment here is aimed to drain dampness
regulate qi
and remove blood stasis. The pathogenesis of the postoperative period or the late stage of stone removal is dampness retention complicated by stasis turbidity
which should be treated by promoting urination in a moderate manner and regulating qi and blood. The pathogenesis of the later stage of stone removal is characterized by excess dampness transforming into heat
combined with spleen and kidney deficiency. The treatment is mainly to regulate the
zangfu
organs
harmonize yin and yang
and reduce dampness and turbidity. In addition
diet and lifestyle adjustment is an important aspect of the prevention and treatment of urolithiasis.
